I belive it should not be related to population. We need more a manageable CCM with Pro-active and knowledgeable people. Fully agrre with your observations.
Our country is quite big 164.000 km2 (4-5 times the Netherlands), but not even 500.000 inhabitants
a) we downsized the CCM from 19 to 12
b) there is no money either to run operations especially to rural areas and the interior where care is needed most...
