Dear Members,I thank each one of you for your contribution and shared learning,it is such good brainstorming and it has enlightened all of us on what is happening in different countries.I would add that each area is a priority only that the starting point differs from country to country.We need to look at,appreciate and adopt a system where each country's pioritized intervention areas should come first.We cannot have blanket priorities for ll countries because each one of them stands at a different level of intervention in HIV&AIDS,TB and Malaria.While some countries have made huge strides in implementation,M&E with tangible impact,others are still lagging behind for reasons I cannot explain.Once again bravo to each one of the members for sharing experiences and ideas...it is very enriching and eye opening..Susan Mulievi...RSA

As you mentioned degree of representation of GF mechanism for
poverty reduction strategies,illiteracy,&income level in
African region,the GF mechanism has CSS(Community system strengthening)
including grassroot solution & health system strengthening for
addressing grassroot socio-economic indicators in developing
countries,though these component are not direct exegetical answer
of above issues but from regional& diseases standpoint number of
countries could address these issues to international forum.The
status of poverty reduction ,functional literacy & income level
issues are primarily & constitutional responsibility of national
& state Govt. of countries.From accountability &responsibility
stand point of countries,the GF mechanism could give formal inter-
national,linkage,financial,pledges&contribution mechanism for
above countries.For addressing above issues in African countries,
inclusive structure of CCM could be answer of oversight problem,
&issues.Discussion of inclusive structure of CCM proposals
development process could address above issues in less GDP&
varied culture countries.

In selection of civil-society as a Principal Recipients of GF grants
in countries,CCM structure should analyse civil-society inclusive structure for addressing GF implementation.From GF standpoint civil-society is consist of NGO/CBO,professional association,&FBO(based on religion & regional overview),this civil-society structure &its
function is not providing complete representation of national consensus,linkage ,community sentiments,social integration& dependency feeling.For representation of civil-society as a PR,
CCM should take various CCM model consideration for correct positioning of GF grants. In preceding discussion, suitable structure of CSO has been discussed for civil-society participation in GF mechanism.

In CCM proposals development process,civil-society could engage
with regional funding mechanism,global trade issues,unorganised
sector employment& agriculture based economies.Degree of GF mechanism
presentation could engage countries to make the CCM proposals development process more inclusive.In developing Asian& African
countries number of countries do not have trade architecture which
is vital component of economic growth in countries.For more inclusive
structure of the above process, CCM proposals developmet process could
involve civil-society participation of specific countries for addressing trade negotiation with greater access to medicines
(socialization of medicines & social mobility factors)for less,
minimum income level,grassroot,marginalised groups(community who
live in disaster prone regions).Civil-society role could be expanded
by making GF trade fair space with awaremes,providing innovative
health care structure,bio-behaviour,capacity building,campaigns,
ethical support,grassroot,rural population for opportunity within
national & international trade negotiation flora.

Dear GF and All participants What priority? If I think, I stands from Indonesia conditions then, all program of GF are very important. GF making good relations with government of cource!. But how GF control all funding, reports, outputs, outcomes which government handling, this is very-very important.I am very hate about corruption in govenment birocratic. I suggest GF must be open to make oppurtunities to create reationships with NGOs. Give their funds to them making direct services to the grassroots. GF must try to believe them. Dear e-forum members,

            We have discussed many important strategies while implementing global funds programs to the affected people throughout the global. The disease which threcend the global HIV/AIDS/TB and malariya are fast growing in the global. During these e-forum discussions gender equality, GF collaboration, communication and co-ordination, civil society and private participation, advocacy, etc are all important to community based organization to successful implementation of any programs GF should assist in solving problems among the agencies ie CCM/LFA/PR/SR in implementation of the GF program through various mechanisms like ,technical assistance, communication, financial and supply of materials. Moreover GF can communicate with the local government and other organizations.

            In the next e-forum, GF may consider the following areas

            Implementation through CBOs by a proper structure with detailed quid lines. Financial out lay for each and evaluation methods. Information, education and communication methods. Training workshops/ seminars related to global fund programs GF implementing agencies status and structures these topic may discussed in the next e-forum but mean time GF should pay more concentration to implementation of the program to fight against HIV/AIDS, tuberculosis and malaria. Be hope to eliminate HIV/AIDS/TB & malaria out of global

Regarding my discussion based on economic model ,I would like to
mention GF Secretariat interaction of various stakeholders for
product mix support,services for fulfill socio-economic expectation
&innovative financing in less GDP countries.These measures could
be applied in your mentioned countries& regions.

The GF Secretariat could provide vital mechanism for (Community-
System Strengthening - Affected population,grassroot solution,
social factors i.e. labour migration,unhygienic conditions,
literacy level,gender disparity factors attributed specific
diseases & integrated approaches.
The GF Secretariat should develop appropriate measures with
inclusion of strategic intervention to overcome oversight problems
in implementation of GF process.
In developing Asian & African countries, product mix,projects &
grants relationship,mobilizing resources are vital issues in
implementation of GF process.In the above process,GF Secretariat
should develop specific product based economic model& procurement
of effective,less expansive products for fulfill socio-economic
expectation of countries.For formation of economic model,GF Secretariat could interact with GF stakeholders in the form of
Public,Private,Govt,economical& financial institutions,research&
development for effective implementation of GF process.
These institutions should ensure product's simplicity,product mix,
promotional mix,hedging,optimum utilization,& integration of
independent variable/participant/channels for mobilization of
resources& grant consolidation.GF Secretariat could develop business
models with inclusion of expertise variable & organisation e.g.
Industry,sales,enterprize resources planning,medical attentive
services. Discussion , requirement of an organisation could give
specific priority of GF Secretariat for mobilization of resources,
integrated approaches& grant making process.

Received reminder.The understanding of requirement of an organisation
is one of the important factor of GF contribution.Number of countries
e.g. African & Asian countries have required GF specific products,
goods& services.GF specific products development could give physical
&psychological support in the form of products(Drugs,equipment,
technical assistance,tangible goods(consumer goods favoured envi-
ronmental factors, sales activity for convenience products & goods)
in less GDP& less market,industries infrastructure countries,these
developmental factors could give strategic focus, intervention &
expansion of specific market opportunity in GF product demand
sensitive& demand oriented countries.In the above development process
foundation,campaign,potential donors cannot enlarge their own
structure in contribution ,standard of conduct could be applied
in above organisational development approaches.
A systematic organisational structure could deal with critical
product dimensions i.e. complexity of product structure,cost&
quality for fulfill sociological,psychological needs of affected
population ,society& community in less market opportunity developing
countries.Organisations should ensure products& goods quality i.e.
utilization associated with purpose,reliability & specification,
physiological& psychological human consideration,instruction(proper-
use) to market& community structure for non functional quality of
products,goods,& services.
Accessibility of GF services in developing & less industrial country
is critical factor for sustainability of GF services implementation,
organisation could ensure appropriate GF services policies&principles
stimulation,building community goodwill, faith& social expectation
by providing products& goods quality with specific standrad of
conduct.
Organisational approaches could help in identification of products&
goods defects,providing adequate analysis& promotional efforts,
time framework for evaluation,cost benefit analysis,demand& viability
of the GF products & goods.
These organisational contribution could give realistic objectives
beyond donor government that contribute financial & other resources
to the GF.

to give reference of civil-society participation including
Faith based organisation .The GF mechanism has given civil-society
component for number of countries having varied social,cultural,
religious,faith values.Above kind of participation could formally
interact with bilateral& multilateral organisation for social,
cultural intervention including (health,education&hygiene)awareness
Discusion civil-society as a Principal-Recipient could give specific priority for social intervention.

From GF mechanism standpoint,Civil-society has FBO component(Faith
based organisation)based upon Christianity community,Muslim community,
& African region overview.Civil-society(FBO) structure could be
expanded by various regions& countries with inclusion of different
religion,culture,society,language&climate i.e. Hinduism,Buddism,
Animism,Atheism,Theravada,Confucianism,Taoism,Folk,non-religious
&Judaism for universal coverage of basic health infrastructure&
implementation of GF principles.
Integration of GF principles with religious,ethical, moral practices,
state & constitutional guidelines could give social integrity,
community sentiments for GF mechanism implementation,responsible
religious practices for mutual dependency feeling,feeling of
sacredness.

GF(FBO)professional association (civil-society)structure could provide
international health guidelines,socio-technical approaches with
appropriately resourced TRP,technical assistance in proposals
development,better use of local resources,partnership strategy in
Tribal,Folk,non-religious community based countries.In the above
communities probability & susceptibility of diseases could be more
than religious ðical practices countries.
In absence of specific religious,ethical practices & due to Tribal,
Folk culture associated with alcoholism,opioid,commonly abused
drugs,Nicotine & meat consumption would have genetically influenced
disorder,adverse health consequences,reproductive dysfunction in the
above communities.In these community,civil-society structure could
integrate with CSS (health system strengthening) for right positioning
of GF mechanism implementation.CSO could scale up its activity by inclusion of specific enterprise
structure support,professional association,research institution,
CBO& formal interaction of governance,stake-holders&various sectors.
If CSO does work like a Principal Recipient then it should have a
formal structure of national & international monetary system,exchange arrangement,convertible monetary units,socio-commercial variable,
effective communication between implementers within countries,
co-ordination with community system strengthening,econo-political
environment,optimum utilization of available resources in countries.

The GF's gender strategy should focus on vulnerability of diseases
for all genders(Men,women,girls,sexual minority).
Accessibility of anti retroviral treatment should depend upon
vulnerability of HIV & lack of basic health infrastructure in
less pharma,health,research & development infrastructure countries.
Above discussion did not evaluate correct assessment of
availability of ART drugs ,treatment& addressing gender equity.
GF gender strategy should be focused on abstinence campaigns,
reduction of high risk behaviour,ethical bio-behaviour approaches,
human development & ecology for men rather than gender inequalities
strategy for women& girls.
Social equity is essential for addressing social mobility &
socialization of medicine in global equity strategy .
Number of social& community factors including geographical,demo-
graphical,religion,educational,economical,biological,&physiological
could require universal coverage of basic health infrastructure&
prevention of specific gender diseases (genetically,acquired,occupat-
ional hazards).WHO has formulated Health for all for accessibility
of all health means to everyone in community structure, ART
accessibility should consider above approaches of an acceptable
level of health to all gender.HIV,Tuberculosis& Malaria are not
gender sensitive.All gender could be affected by these diseases so
prevention method & treatment should be equally provided by GF
mechanism & strategy for social equity & justice in community system
strengthening.Human health is consist of biological,social & psychological concepts, if degree of prominent risk factors are
linked with diseases than GF global equity strategy could develop
an independent component of accessibility of ART & other treatment
for men in community system strengthening alongwith affected popu-

improving sexual minorities health services understanding &
accessment of socio-economic structure & conditions,universalising

In developing world above factors are responsible for discrimination
in sexual minorties.
Sexual minorties term & conditions could be eliminated by using human
development program including national GDP,standard of living,
literacy rate,gender related development & sexual minorties
empowerment measures.
National understanding of gender's right & empowerment(health,education & nutrition)can give economic & social prograss
for sexual minorties.
Strategic interventions in the area of social &structure opportunities for sexual minorties could eliminate social discrimination.
In developing nations addressing religious & cultural malpractices
are challenges.Vulnerability of HIV/AIDS,stigma,discrimination
could be eliminated by addressing disadvantages of religion&
cultural based malpractices in many countries.
Above factors should include gender empowerment,rights,livelihood,
human ecology,socio-legal prohibition on inhuman approaches causing
sexual minorties,awareness of science,bio-behaviour,social-
acceptability,universal benefits,cultural diversity,national&state
HIV prevention program accessment.
Social acceptability is also a challenge,can include national health&
services support by providing employment,trade,grassroot work & shelter for sexual minorties.

Dear Forum members:

1-There is a need to strengthen GF mechanism for effective programmes implementation, cooperation and communication with stakeholders reforms to be focused on better funding mechanisms, wider involvement of stakeholders, partnership collective responsibilities in data gathering( Monitoring& Evaluation) on GF operation

2-Development of more collaborative and coordinative system.,  Partnership should focus on Government , NGOs, Private sector,& Civil society organizations chosen as PR by CCMs to better monitor other NGOs.

3- GF should restructure its administration and consider  establishment of regional offices  of Secretariat for better implementation of programmes

4-Ensure that indicators can effectively measure the contents of proposal.

5-Improve and better communication BETWEEN CCM and secretariat.

6-For specific gender intervention , there should be understanding on country community, cultural and constitutional understanding in implementation of programmes and should take ethical approaches (social, economic, health, educational empowerment according to country norms and constitution.

7-As a priority GF should focus on prevention programmes and increase public awareness against three diseases . It is of paramount importance that GF to give adequate emphasis on health promotion and and behavior change communication for prevention of the three epidemics rather than all GF attention  are directed towards curative medicine.

8- Special efforts  should  be directed  towards adequate Malaria surveillance data and to  reach “ Malaria without Mortality “. Beside country-based approach , there should be regional monitoring laboratory and surveillance  networks that support country efforts , apply standardized monitoring techniques , rapidly share finding and manage coordinated response which would aid in saving billion-dollar malaria effort is flying blind.

9-GF should support activities that bring  long lasting changes in health efforts by encouraging institution and capacity building and introduce models of public health and health care that can  take role for future  better improvement of the society as a whole not only three epidemics.

10- GF should support developing countries most aggravated by diseases to balance their needs and ability to handle diseases and give more support to poor countries and strengthen their technical and administrative staff for better implementation of programmes.

HOW NGOs, CIVIL SOCIETIES, COMMUNITY BASED ORANGANIZATIONs etc SHOULD WORK. There is a lot of work and principles to follow. The NGOs or others groups should be people that have human sympathy. Their aims and objectives is to help lives, giving hope to people not looking at race, culture or ethnicity because every human being have equal right to life. These groups should present well organised programme from the grassroots before GlobalFund should grant them funding. My approach to this effect is as follows; for example, WISDOMWORLDSOLUTION should call on the local government areas and get all information about their communities and villages. In each village, they should volunteer two people that will represents them, man and woman. They will undergo training about these three dangerous diseases which includes prevention, treatment and Caring. They should know what affect them most and what they need. However, these two people from each villages will go back with useful tools having established contacts and starting working on the field. These action will give more light to Globalfunds to the grassroots because these implementation will change their standard of living to more promising future. Global fund by then have reached their aims and objectives and grassroots people will know more about the importance of Globalfund. Finally, Globalfund should look into the programmes of the NGOs , CIVIL SOCITIES, COMMUNITY BASED ORGANIZATION that affects the grassroots before giving grants. This is a practical concept not theory. I choose these implementation because all purpose and principles not well attained its action is a failure. One of the greatest concerns for us in developing countries in the years to come will be whether the funds will continue to be available five years, ten years from now. Hence, priority should be towards ensuring the sustainability and predicatbility of funding.====================================================================================================================================================================================================================================================== While GFATM will naturally and instinctively gradually evolve to address the myriads of challenges as have been brought to light by numerous contributors to e-fourm discussions, the GFATM should work particularly work out modalities towards "investing" funds put in her trust with economic efficiency. CSOs and other implementers should be made to work strategies that will ensure that the programs that they are asking to be funded are sustainable. The Rolling Continuation Channel should be abolished and other incentives for continued funding worked out. The onus of supervision lies with the GFATM secretariat while that of working out socio-entreprenuerial innovations, to ensure that this programs are sustainbale, lies with the grant recipients and implementers, whether they are CSOs or whatever. It is possible to work out something that will ensure the effective and efficient use of funds, if we will all put our minds and heads to it.========================================================================================================================================================================================================================================================== The funds that have been poured into funding public health programs focused on the three diseases, if it had been strategically invested into health systems would have been able to make a lot of difference rather than being continually being poured into the 'waiting beggarly cupped hands' of poor and developing nations. Such innovations will be the catalyst for lots and lots of other benefits that will be lead to numerous other benefits, among which is the reduction of the brain drain to developed countries of the best trained health care professionals from poor and developing countries and as well a reduction in the complexities in the GFATM grant approval process and in the poilitics and intrigues in the appointment/nomination of PRs/SRs by the CCMs. Let PRs be appointed based on innovation and economic merit or proposals and not based on political horse-trading, intrigues and sentimental activism.===================================================================================================================================================================================================================================================== On the last issues that I pointed out, a lot of the other challenges that have been pointed out by contributors on issues such as demand, CSOs' adoption as PRs, partnerhips and implementation, will be effectively and efficiently tackled if grants are approved based on the merit of economic viability and sustainablility of programs. That should be the criteria for approving grants and that should be the criteria for the selection of PRs/SRs by CCMs.======================================================================================================================================================================================================================================================= CSOs should not be appointed PRs based on political sentiments and activism but should evolve into Socio-entrepreneurial entities that will give a good account of the funds made availbale to them and as well utilize the funds with the same efficiency and effectiveness of blue-chip companies. However, a fundamental difference here will be that these 'socio-entreprenuerial organisations' will not be particularly be motivated to be in business to maximise profit but will have the interest of the poor and vunerable end-beneficiaries in mind. That should also apply to every other organisation that intends to be appointed recipients of GFATM grants. One other benefit here is that CSOs will be forced or rather be motivated to build up managerial structures and capacities that is required of all those that aspire to become recipients of GFATM funds.======================================================================================================================================================================================================================================================= There are many benefits that will be derived, which are as many as the myriads of challenges that the GFATM is currently facing, if the GFATM e-forum will focus on the above mentioned issues that I have raised. I must recommend Globalfund for her intelligent planning to bring about success in their programmes designed to fight against HIV/AIDS; TUBERCULOSIS AND MALARIA in different countries of world. However, to give more light to my suggestion about implementation and how to put it into practical work. In every country there is a link between the government and the grassroots people which are the poorest and where these funds are highly needed. Let me explain how it will reach them and change lives which their government have not attained since the creation of global funds. In every country there is a State or region and every state or region has Local government then all local government has Autonomous communities. In these autonomous communities is where the grassroots are found , they are called Villagers. In some villages there is no electricity, no clean water ,no hospitals or clinics, no schools and people are very poor. Moreover, this is the sector where you will find people dying because of one dollar( $1) because they have no money for hospital in the towns also no transport fare even emergency. There is no schools in those areas, most people are illiterate and do not know what is called HIV/AIDS, Tuberculosis and Malaria, how deadly these diseases are until they are affected. Hence no treatment so they die. It is very sad and sorrowful. Meanwhile, this is were their government has failed in many years and the worst is that these people are forgotten. They do not know about Global Funds so the only people that will help them are the NGOs, Civil Societies, Community Based Organizations and some private individual. Government assistance are inaccessible in these areas. At grassroots level is where Global Funds should focus before granting funds to NGOs, Civil Societies, Community Based Organizations etc and when these are archived then the world has become a better place for all. Dear e forum; GF portfolio in its present form is inappropriate and difficult to manage and control.  In particular, the wide and disparate activities carried out have tended to diffuse coordination capabilities. This position has led to inability to prioritise key GF project between HIV/AIDS, Malaria and Tuberculosis’ projects implementation and sectoral seminars/workshops or general GF administration. In addition, the GF’s wide variety of projects have been inordinately difficult to organise, prioritise and monitor. Each type or category of project really needs although has not had criteria which are specified to suit the prioritisation and selection necessary for effective allocation of sectoral resources against requirements. At present the same criteria are applied across the board which of necessity, has led to the selection of GF projects on an ad-hoc basis. To counter-react this major weaknesses, I would recommend that the GF project portfolio should be classified into at least four major categories a long the following lines; 1) HIV/AIDS, TB and Malaria ( this should represent all direct GF capital projects)., 2) Research and Development(this should cover all research related GF projects and studies as a sub-set under the overall main heading); 3) Resource Development(to represent projects covering such areas as training and support services and equipment aimed at strengthening sectoral capabilities), 4) Administration Support( this should be sub-divided into three areas as follows; Direct secretariat support, Sectoral co-ordinating unit and GF meetings and conferences. Such a framework would require that all current GF projects are re-classified, under the categories as suggested above. In addition, objectives and supporting strategies would relate more closely to each category of projects. The development of Information Systems should also be based in part in these categories to support the planning and control of operations at every stage of each GF’s project life cycles from inception through to completion. In conclusion GF should develop and adopt an information systems strategy as a matter of priority which is focussed upon clear definition of information requirements, a consistent means of data capture throughout the organisation using a database approach, and selection of common hardware and software for use across the GF organisation. I believe that this strategy would facilitate significant improvements in GF management, planning, co-ordination and control of prospective, approved and live GF projects. Remember "to manage business is to manage its future, and to manage the future is to manage information"

I have been following the comments made in the e forum and agree with some and disagree with others. I did disagree with the earliest comments about Mark’s remarks in the Nature Own Magazine about Malraria programmes being blind. If you do not have an original baseline – and a lot of Malaria endemic countries do not, then we are all flying blind. Thanks to Global Fund I am seeing significant reductions in Malaria across the board. In a few months time we will have achieved 100 percent coverage of the population (using 2.5 persons per net, which we now acknowledge is too high) and we have seen a significant reduction the morbidity and mortality of Malaria. Without the Global Fund it would never have been possible and we would be seeing Malaria deaths get into the five figure range as our Highlands provinces get affected. Oh to be able to comment from a country that is Malaria free.

Fortunately in Papua New Guinea we have a good gender balanced CCM. This is very commendable in a society where the female is considered by a large majority of Papua New Guinean males as being simply “Property”. We only have one female member of parliament but it is a start. Regrettably we have very little Private Sector participation in the CCM because we have a limited number of spaces for private sector as most of the 26 CCM positions are taken up by the NDOH, multilaterals and bilaterals. In saying this we do have tremendous Private sector support for HIV/Aids and also for Malaria. TB is still a bit of wait and see from the private sector.

We have very good representation of civil society on the CCM but unfortunately they are not particularly vocal and I, as Vice Chair would like to see the capacity of Civil Society Members increased so that they can actively participate. We will hope to get some field trips to visit CCM’s where civil society takes an active role and are as vocal as the other CCM Members.

You are so right...WISDOM WORLD SOLUTION: To circumvent incountry high level corruption, the Global Fund must decentralize all its operations to open regional offices. They must also have national representatives--- of a different nationality, but who understands the country's cultures and tricks of the trade as monitoring agents of how the funding disbursement trickles down from the Principal Recipients to SubRecipients until it trickles down to the grassroots. A risk management system of spotchecking and strict quality assurance needs to be in place. This GF country representative also must be in charge of building capacity to attract civil societies, Faith-Based and Community Based OrganizationS and to assist them to meet the criteria to receive funding. It should be the goal to qualify as many of these marginalized grassroot organizations so that quality care can be delivered in the context of the people's cultural beliefs and worldviews. May I remind you that in order to successfully sustain the strategy you eloquently presented, which is to prevent, treat and care, we must build capacity among these poor people and make them functionally literate. This is the only way we can retain the gains we have made. Other donors can provide poverty reductions programmes that will further enhance their quality of life. If we leave out the functional literacy strategy we will be doing ourselves the greatest disservice and worse yet we will deepen the marginalization of the very people we should be reaching out to with open arms.
