Dear GF future Scenarios. I have hihglighted below what Gf should look like in 5/10 years time, what should it be doing, How should it be doing and \who should be involved.

2.       WHAT SHOULD BE DONE AND HOW THIS SHOULD BE DONE   IN THE PROPOSED SET UP

 

2.0     ADDRESSING THE DIRECT AND INDIRECT CAUSED OF           HIV/AIDS

 

This should be done through main streaming HIV/AIDS in sectors and National programmes. Many HIV and AIDS responses to date should address the direct causes and underlying causes of vulnerability to HIV infection and the longer term consequences of HIV/AIDS. Not all organizations are expected to take on the same task and responsibilities. Rather, mainstreaming means integrating HIV and IDS into functions relevant to the core mandate of each country sector and entity. Thus mainstreaming in practical terms may differ between education and agriculture, mining and the media. However, there are certain strategies and actions which are common to different sectors reducing vulnerability to HIV infection and mitigating impacts of HIV/AIDS.

 

Lessons learned and knowledge gained from each country and others in the past two decades have shown that the underlying causes of vulnerability to HIV infection and impacts of AIDS are systematic and structural as well as influenced by societal values and cultural norms. To turn the tide of the growing epidemics, responses must go beyond the direct to confront and respond to the indirect factors that perpetuate vulnerability to HIV infections. The table below provides examples of some of the key indirect factors and suggestions of possible responses.

 


 

HIV VULNERABILITY FACTORS                  POSSIBLE RESPONSES

·        Fear, denial, stigma & discrimination         - Provide awareness education in schools, out of schools, at work places and promote understanding through media – based advocacy campaign and community outreach efforts.

·        Gender inequality & power differentials      - Involve both men and women to ensure equal access to education, employment and protection of human rights in addition to promoting an enabling environment for negotiation of safe sex with partners.

·        Poverty and livelihood-insecurity                  -Take actions at local and National levels especially for the remote or poor area by mobilizing and promoting local talent and resources to alleviate hunger and reduce unemployment through sustainable means, provide financial assistance to the affected and infected individuals with clearly laid out and feasible action plans.

 

VULNERABILITY FACTORS                           

·        Migration and displacement                        - Implement policies and programmes to enable access to health care and workers’ protection especially for migrant workers both domestic and foreign while ensuring safe work places.

·        Social-Cultural norms, values and practices                                                     - Place HIV and AIDS issues in social, economic and security context of a society, proactively engage people living with and affected by HIV/AIDS to help the society understand which norms and behaviours are potential factors to increase the risks of contracting the virus. This is region specific and depends on the regions and values of each community.

·        National legislative and policy environment          - Enact National laws which support mainstreaming effects at all levels. Workplace policies and community based awareness and treatment support by programmes should be supported by legislation to ensure their effective implementation. Engage objectively focused politicians to take an advocative role on HIV/AIDS based on the National commitment in considering HIV//AIDS as a national priority given to its threat to National security and a crisis with international implications.

 

 


 

3.                 WHO SHOULD BE INVOLVED AND HOW?

·        The Organogram on page 1 shows clearly how the proposed New set up should be but in addition these are some of the salient details:-

 

v    Technical and management capacity of the project implementation teams:-

-         The Country Focal Contact Point in direct collaboration with the Geneva Headquarters will identify reputable, transparent, capacity based and experienced civil society organizations to implement the project through their equally identified community based organizations throughout the country.

-         In order to reduce direct administrative burdens Country Focal Contact point in collaboration with Geneva Headquarters will approve experienced CSOs (National) and Region based with recommended working relationship with the district governments in context; to work as LEAD AGENTS for Global Fund Project. i.e. CSOs should be brought on board to work for the 5-10 years contract renewable depending on satisfactory performance.

 

These regional Lead Agencies will be responsible for the following:-

 

·        Building the capacity of all the community based organizations and that of local government staff at Sub County levels, in aspects pertaining to HIV/AIDS – prevention, care and support of the affected and legal protection.

·        Release project funds to the CBO/NGOs proved by the CFPC.through the Regional Lead Agencies for project activities implementation at Sub County and community levels.

 

·        Account for all project funds either disbursed directly on the Regional CSOs’ Account by the Global fund in Geneva Headquarters or that disbursed on their Account by Country Focal Contact Point which they in turn disburse to the CBOs/NGOs Accounts at Sub County.

 

·        The co-ordinate the project activities at Sub County level through reports submitted to their offices by CBOs/NGOs trained (TOTS) staff to partners as shown in the organogram.

 

·        To supervise monitor and evaluate project progress and give feed backs to the stakeholders as shown in the organogram.

·        To represent the views of all target groups within the CCM meetings by ¾s which can be clearly received by the CFCP the Global fund headquarters Geneva respectively. Co-ordination mechanism in this proposed et up can also be done by officers from Geneva headquarters to the Regional CSOs depending on their wish.

 

·        To establish and develop linkages with the existing service providers strengthen working relationships with Districts and Sub County local governments’ personnel and the private sector across all sectors in development.

 

 

 

COMMUNITIES

 

Communities are our targets and are composed of high risk groups specifically outlined within the organogram on page 1.

These should have the following roles:-

-         Identify their group leaders and select working teams to develop development plans in a participatory manner.

-         Receive technical skills for implementation of the planned an approved project activities.

-         Set up participatory management committees for their proposed projects for sustainability.

-         Implement project activities as scheduled according to time frame.

-         Suggest possible modifications of project activities where the initial plan has been environmentally incapacitated.

-         Monitor the project activities’ progress in a participatory approach and report accordingly as shown in the organogram on page 1.

-         Identify social cultural beliefs which continuously culminate into HIV infection but can communally be discussed and mitigated without affecting cultural beliefs i.e. using the win win paradigm.

 

NB:      MAINSTREAM HIV/AIDS IN DEVELOPMENT PROCESSES

In order to share HIV/AIDS funding across sectors the New approach suggested above puts into consideration the fact that there will be a dependency on individual sectors reducing the burden on the Ministry of Health. With the guidance of the CFCP, all sectors i.e. CSOs in the Lead will identify sector specific HIV/AIDS interventions within their mandate and present their proposal indicating the resource requirements to Geneva headquarters either through the CCM or directly to the Geneva headquarters. CFCP should follow up and make necessary clarifications with the Ministry of Finance and the Geneva headquarters before funds disbursement to CSOs is finally made.
