I feel that all sectors should be represented for the wave of healthcare is changing with the dawn of the technology age. I challenge the public sector, non-for-profit, and academic to stop looking at corporate as "Big Bad Business". In particularly, those in the HCIT arena are engaged in research to improve the community- domestic and globally by translating HCIT into practice. In health system strengthening it is best to leave technical leadership up to WHO. With one exception. Some in WHO are pushing for universal HIV testing in high prevalence situations. You can read my comments on this elsewhere through this link:

This is one area where the Fund may have to exercise leadership. The Fund should not fund activities that violate human rights.

Health Systems are weak in most African Countries according to reports. A weak Health system has contribiuted to the failure/ delays in the implementatuion of "Excellent" Health Policies. In Nigeria for example, the Federal Government has tried to lay the necessary framework for Health System strengthening. Successful public health interventions must stand on a functional health system. My experience with the implementation of the GFATM Malaria grants shows that a major constraint has been that of weak Health systems.
However health systems need to be strenghthened with due attention to the socio-cultural and demographic issues. Hence it is my thought that Health system strengthening should form a part of the component of the 3 major diseases. The issues will include Advocacy, capacity building of stakeholders and community moblization among others. I have also observed that most communities respond to interventions which have been planned with their inputs.

As long as the battle to fight the 3 diseases, AIDS, MALARIA and TUBERCULOSIS is still going on, better to go ahead with the three while collaborating with those who are committed to others diseases.
In my opinion, it is good and wise to maintain the cap on the THREE but if the budget allows, the GF can assist in other fields accordingly of both: budget and need .

Move from vertical to integration where everyone will be involved ie General Health Staff, Community leaders, Imams, Church leaders and ordinary man woman in the community. This will encourage ownership.
Promote accountability of recipients not by paper presentation only but by being audited by staff of the Global Fund WHERE PEOPLE WHOSE NAMES APPEAR ON any expenditure are confirmed by the staff of the Fund.

The Global Fund should involve other funders in supporting health systems strengthening and as a seperate function from the funding mechanisms of the three diseases. The Global Fund should lay more emphasis in the areas of capacity building for service providers and managers of the health system and in the area of monitoring and evaluation. Most health system operators still find it difficult to track projects and report accurately outcomes and impact indicators in evaluation of projects. Health Systems Strengthening goes hand-in-hand with most other goals the Global Fund is trying to achieve and therefore cannot be ignored. For example, the goal of universal access to treatment for people living with HIV and AIDS will never be attained without further strengthening of the health care system and the human resources operating these systems.
In South Africa many qualified doctors and nurses have left the country for work in places like Saudi Arabia, the UK and the United States because of better salaries. The goals of the National Treatment Plan there have not been met and this can partly be attributed to the overall capacity of an overworked health care system and limited human resource capacity.
Health systems strengthening should be mainstreamed into funding for the three diseases because of their close association. The Global Fund should also help to fund fair salaries for health care personnel to ensure that adequate human resources are available to provide much needed care, treatment and prevention services and to ensure that the Millennium Development Goals are met.

Nowadays, it is almost impossible to separate endemic diseases control results from the performance of the health system as a whole. Nevertheless, I think that the GF funding component should be separate (answer a.) also including health systems research, due to local/regional special features of organizational issues and different endemic levels. Besides this, there would be better possibilities to develop and implement combined or integrated approaches among the three diseases or even with other prevalent ones, according to the local epidemiological or operational situation.

For health system strenghtening you need a health system. A health system needs human resources. And...these human resources need to be paid. WHO recently pointed out that the first requirement (the presence of HR) is sometimes problematic (how to keep qualified people in Sub-Saharan Africa?). I believe that to keep human resources, they need to be (well enough) paid and motivated. Once your health system has motivated, aid HR we can start talking about all the things mentioned in your introduction (Advocacy for political and civil society support of certain strategies, programmes and interventions, building communication capacity, building community and district capacity to participate in strategy and policy formulation, general capacity building for institutions and in management, accounting, procurement, etc., undertaking research & enabling international standardization, developing computer based learning and information exchange tools to improve health service quality, training and maintaining high quality healthcare service providers, and building monitoring & evaluation capacity,...) I think other donors should be encouraged to work harder as well as team up with Global fund for a better working relationship.Also, those not contributing now should be encouraged to do so.There other companies wanting to donate but do not know how to go about it.
Secondly, harmony will surely encourage as well promote Global Fund goals in either curbing or eradication of all the diseases its programme are focusing upon.
Lastly,usage of local languages at the local level will definitely boost GF impact as well reduce some of the stress attached to it.

In our opinion it is better to have a health system strengthening funding component same as the three funding components of aids malaria and tuberculosis but the proposals should adressesd directly to the grassroot level not at the country cordinator because these man are not fare enough to reach the messages in our villages where there are a lot of victims. They almost engaged in town areas.

Appproach (a) is necessary. The Global Fund needs to allocate separate funds for Strengthening Health Systems. Priority areas for strengthening may be selected in consultation with country institutions. Health Systems Research Capacity Building may however be supported in all countries, since this will provide reliable evidence for the monitoring and evaluation of all health projects and programmes.

This should ideally be done separately, since the issues surrounding HSS are not sequestered to AIDS, TB and Malaria
We all acknowledge that a weak Health system can not provide an effective reporting structure that the funding needs, leave alone provide for efficient use of the resources deployed (Commodities included).
In Hind Sight this may have been the first thing to tackle before deploying materials and resources.

Approach the associations /organisations which are serving in the same feild for rural places or you can directly be involved in the same.
