    I am afraid that really global fund is being ever disseminated to grassroot NGOs in developing country. There are many subjects like safe blood HIV/AIDS in remote areas.

    Proper Project Planning and a good Network Infrastructure- these are two steps that the Global Fund can take to be more in line with country priorities and systems, therefore reducing transaction costs and ensuring the lasting impact of its funded programs.[/u] I work amongst the tribal people (High Risk Zone for HIV/AIDS, T.B.& Malaria) and the poor sections of the society. Apart from being poor these people are slowed by illiteracy and superstition, making our job more difficult to accomplish. To build up a sustainable future for these sections of the society, the first and foremost requirement is to make them self-supporting while, at the same time, spreading education among them. But the main hurdle to achieving this goal is the altogether absence of any infrastructure for this purpose. It is, evidently, not possible for some individual efforts to build up the necessary infrastructure for the same. In this context, I very much feel that it is only possible through the Global Fund Partnership and support.

    Background:

    The Global Fund was established to be a financing mechanism that would fit with existing country systems and impose minimal transaction costs (defined as requirements on administrative systems, implementation processes and personnel in working with the Global Fund) on the countries it funds. Furthermore, it was designed to be an additional source of funding, complementing the efforts of existing donors. However, after four years of implementation, it has become clear that some areas of the Global Fund’s financing model (including the rounds system and the Comprehensive Funding Policy) and architecture (including CCMs, PRs, LFAs and the TRP) could be improved.

    If harmonization and such hurdles can be discovered and eradicated anywhere, I believe it will be with the Global Health Fund. By it's very nature solutions are sought out and hurdles identified at the most base level from those on the ground with the patients. The recalibration due north with the facilitators of this fund is something to be admired and hopefully an example and template for governments and economics of the future. But I digress.

    It's been mentioned earlier by Dr. Mambo I believe in regards to defining specifically what the priorities of each country and region are. I agree. But then to come to a reasonable compromise if necessary, and plan and manage the project directly from that spot. Sounds simplistic maybe, but harmonizing should be. It gets muddled down when simplicity evolves and evolution is not always for the better.

    Just like children, every project is going to be different. . . .some will function fine and above board independently and some will need constant monitoring and correction. Could area managers or administrators become the go-between between the nurses on the ground and the ministers in the capital? Does the GF provide ambassadors of sorts to parent the grants they supply? Another middleman does not sound very appealing I know . . . . . there are already plenty i'm sure with their plates full. But in light of trying to account and streamline time and assets, and here we are hearing from concerned facilitators on the ground who are disturbed that the purse is not full or fair. . . . .

    Some will complain when it comes to accountability - time-consuming paperwork, recordkeeping, "why again is this necessary?", but fund providers in the past, including us taxpayers in the west, have seen well-intended funds decorate presidential palaces. No one likes to open childproof pill bottles or see stamped messages on plastic bags stating they are not toys, but they are there due to legislation and necessity for the least common denominator of assumed common sense. Or lack of education or experience, or a reminder we can't turn away from the little ones for a moment without potential for hazard. Proper parenting may be in line for these funds.

    It is good to work with a variety of stakeholders to have better impact and reach, in line with the country priorities. Global Fund should work with Government so that it may be in line with country priorities and can also influence policy. At the same time it should work with NGOs and civil society to innovate and develop models, which some times may not be possible by working with Govt alone. The best thing is networking and developing stakeholders’ platform for mutual sharing and learning. The lead role should be given to the government because the fact is that in terms of reach many developing country governments reach more people than other programmes. Making best use of the government infrastructure and facilities and making them more accountable and efficient by working together will be more sustainable.
