The best thing to measure a performing CCM is the following
 
1. First and fore most measure the performance of the grants - are reports coming out in time, are they of good quality, are the targets being met?  Though it does not implement, the oversight role can only be measured in the grant performance.
 
2. Another way to measure is the quality of proposals being produced.

To, CCM(Performance framework) Well performing CCM should have clarity of constituencies responsibility i.e. an elected member /CCM representation from specific constituencies should be accountable for strengthening of community & health systems & not produce problematic situation for an affected population, which is very vital addressing for CCM at grassroot level in countries. For an inclusive proposal development process,constituencies responsibility& affiliation could give consistent solutions support for intervention in prevention of diseases & reduction of discrimination at health facilities. An effective CCM could use constituencies functioning for prevention of refusal,isolation,& discrimination af an affected population on socio-economical fronts i.e evaluation of health expenditure, various sectors suffering(value added terms)general health protection measures, analysis of constrained factors attributed diseases& vulnerability for an affected population. To, (Performance framework) Performance of CCM ,might be suffered in less trade infrastructure countries i.e. less trade issues,unorganised sectors,ineffective participation of CSO/NGO/FBO & less food security.For an effective CCM performance,country's trade architecture& agriculture based economy are vital components in country, addressing consistent GF mechanism. No. of economical aspects& specific intervention in prevention of diseases i.e. cost sharing proportion & eligibility,trade negotiation with more access to medicines(socialization of medicines & social mobility factors could be influenced by national,public& private , socio-economical strategies.Well defined CCM should consider OECD criteria for understanding of socio-economical factors & perform monitoring & evaluation functioning to PR& OPR. Well performing CCM should have PRs & SWA in less GDP & middle income countries with less opportunity of international trading system. Less & lack of trading system could cause high pricing & ineffective national income support.These factors could aggravate specific diseases situation & impoverished population in countries, involvement of deliberate & incompetent methods of economy could not define interpretation of transparency & meaning of participation for impoverished population in various countries.

According to '"have not" situation in countries, an effective CCM should develop contingency model addressing objectives in reducing mortality & morbidity associated with impoverishment & specific diseases. CCM cotingency models could consider bilateral, multilateral, multisector approaches, diseases specific committee & GF trade opportunity for impoverished population in various less GDP countries. Discussion e-forum based on effective mechanism could detail predictable CCM contingency model for impoverished population in countries.

In implementation of GF mechanism,an effective CCM should understand sociological factors including culture, language, climate, social adaptability of goods, services & differ economical factors including commercial policy variable, assessment of international monetary rules, international liquidity, foreign exchange, multi exchange rate, production factors, capacity of domestic industry for procurement of goods & services. GF could give effective mechanism with inclusion of different sectors.

From GF standpoint, CCM, GF secretariat, governance could expand their own structure by inclusion of agricultural sector, processed products, non-engineering, domestic industry for participation, contribution & stakeholders communication.

Above mechanism with different sectors could encourage the partnership for joint capcity development, procurement of goods, products, services & commercial co-ordination in less GDP & less health infrastructure countries at the regional level. Inclusion of trade fairs & trade opportunity i.e.establishment of GF trade fair provisions in Fund raised & spent component of GF mechanism could give trade communication & trade opportunity in less industries countries for global accessibility at the regional level.

CCM, whether it is a process or mechanism, has definitely challenged the conventional decision making process where government (most often a bureaucrats) makes a decision and civil society accepts it. At the CCM meetings, it is often an uncomfortable situation to many personnel both from civil society and government agencies to sit together on an equal footing and make joint decision. Nonetheless, gradually many are learning to fit to this new dynamics of decision making process. Despite all these process and practices, there is no evidence that CCM decision has influenced government policy, but government policy has definitely influenced the CCM process and decision.

Despite many CCM being in place and operational for more than 8 years, this mechanism is not yet adapted by other funding agencies, therefore this mechanism is still under scrutiny of many. CCM has a very specific purpose and role vis a vis Global Fund programme. It should be however recognized and also appreciated that CCM has overwhelmingly dominated HIV/AIDS implementation mechanism at the country level and the discourse around it. Whether such domination is resource laden or guided and shaped by nature of epidemic could be a subject for further debate.

CCM neither can substitute any existing ministry or department nor replace the functions of such established institutions. Therefore the performance of CCM should be viewed or measured within the limits of this boundary.

With this background, the performance of CCM is best when

o        its members are democratically selected by their constituency

o        government representative do not impose decision

o        members do not use CCM as a forum to advocate their individual interest or bargaining platform rather looks into the issue from national perspective

o        the member do not influence PR/SR selection favoring one or other organization, rather agree a criteria and ensure that it is adhered to

o        the member go back to their constituency (or respective ministry) and share the outcome of CCM rather than criticizing the CCM decision

o        the members own the CCM decision and are accountable to it

o        professionalism prevails over politicking

o        GF secretariat do not micro manage and challenge/disprove the CCM decision

o        it or its member do not wish to manage the implementation rather oversee the implementation using agreed standards and criteria.

A well performance CCM have to be a good character I each dimensions. It should measure qualitative and quantitative in all indicators openly and transparency. If there is any need or Assistance to the partners of GF, CCM have to come forward voluntary to solve any problems / constraints raised with in or outside the GF partners. The CCM have to facilitate all needs (ie., technical, preparation, planning, training, dissemination and implementation of the G F programs) to fight against three diseases.

The CCM must be an antonymous body, it should discuss with the national health program for non- duplication of the programs implemented by the G F partners. There must be a transparency accountability clarity in planning & developing and openness in policies in related to G F programs.

CCM have to ensure the better implementation of the G F programs to fight against three disease HIV/AIDS/TB/Malaria.

Performance of well defined CCM could be measured by comparison of CCM specific project oriented standard performance & actual performance achieved by involving all components of CCM. National M&E & an independent monitoring & evaluation committee could assist in oversight functioning & identification of the factors& causes of GF implementation deviations during grant implementation. CCM components& members should be communicated for corrective action during implementation, analysis of deviation for enhancing performance i.e. demand specific solutions,identification of possible obstacles, partner environment,review proposal submission,multisector inclusiveness,representation from all vulnerable populations&diseases specific committee.It is very impractical & irrational to carry on the affected population or vulnerable population audit,country CCM should ensure proper monitoring & regular evaluation for PR & OPR in order to achieve the country proposal objectives & serve all vulnerable population in countries.

There are pointers through which CCM can be assess, some of these characteristics are:

  • Transparency and Accountability
  • Involvement of all arms/stakeholders in planning and implementation of programmes.
  • Information flow which will lead to result attainment.
  • Proper coordination of activities.
  • Efficient and Effective Monitoring & Evaluation Mechanisms in place etc.
 
How to Measure Good Performance

  • Result will be attained.
  • All Parties/Stakeholders will be interested to contribute towards Goal attainment.- Accessibility of information,communication and reporting as at when due.

A well performing CCM should have the following;
  1. One that is participatory in which all the key players,including PLWHIV are involved.This will ensure that all the details will be bone in mind throughout  all the project cycle.This will also ensure ownership and long term sustainability of all projects;

  2. They should be impact oriented,both qualitative and quantitative...this will ensure that projects that are implemented achieve both the overall and specific objectives as pre-stated.Follow up monitoring and site visits would add value;
     
  3. There should be plenty of oppeness and accountability to all key players,the beneficaiaries and communities included.This will ensure that all the stakeholders are aware and participate in all projects activities.Everything about PLWHIV must include and involve PLWHIV;
     
  4. There is need for a spruce up in the coordination at national levels and awareness creation of CCM activities and calls for participation of organizations and individuals  through tools and media that reaches even those who have no access to the internet.This I say because I work in country where CCM is working and yet some organizations in the HIV&AIDS sector have no idea of what  CCM is and does.

CCM's have multiple roles most have already been raised. I would like to add/highlight some of the key roles that I see as lacking:

  1. Coordinating with National Planning bodies to assist the policy level changes and awareness needed for the long term solution of the problems addressed by the GF support grants, this could be on policies related to access to treatment of marginalized groups, or long term budgetary support;

  2. Assuring transparency in the specific roles tasked to the CCM, such as the announcement of proposal applications, the selection of PR, SR and the proposal formulation process coordinated by the Technical Committees under the CCM. Transparency refers not only to access to information, but also the processes used that need to be consistent and written out clearly;

  3. Transparency measures should apply not only to the CCM but also the technical committees that the CCM sets up. It should be clear when a PR or SR is sitting on the CCM or on the technical committees, that there is no conflict of interest;

  4. Commitment of CCM members to make available time to understand the programs that are funded by GF, and not merely to attend CCM meetings.

A well-performing CCM should be characterized by following features:

  • it has a strong mandate to issue policy measures necessary for public health and three diseases, not recommendations for national and local governments that are not obligatory and can be ignored;

  • its composition shows a balanced representation of all local regions, communities and institutions that are dealing with the three diseases, as well as international donor community;

  • membership criteria are transparent and allow for all the above communities, regions and institutions be represented;

  • it has a necessary organizational capacity to generate and pass the proposals to Global Fund for each round for each disease component that is relevant to a given country;

  • it has well-established and transparent mechanisms to select new members and rotate old ones;

  • it demonstrates mechanisms and particular cases of overcoming conflict of interest;

  • it is measured by either a government body or a non-government watchdog on all of the above and other principles and results of the syrveys are widely known;

  • country proposals get funded;

  • unfunded proposals are scrutinized and lessons learnt;

  • problems that arise in the process of grant implementation, can be overcome by CCM decision enough mandate;

  • Citizens of the country know about its existence.

Forum Member Rele and practically everyone else has emphasized accountability and transparency as the essential features of a well performing CCM. No disagreement here. Hopefully CCMs know how to demonstrate these characteristics. I also assume, as Member Sawran El-Sheikh has pointed out, that CCM’s main task should be to review progress of the implementation of the approved plans.

believe a well performing CCM would facilitate review of progress on key indicators for three disease areas by having data charted graphically and discuss it in each meeting. If not, may be this is a small gesture that could be added to the ritual. In fact, as Member Jim Campbell has pointed out, additional charts can be included in the display on other diseases or compatible public health measures to show comparisons. This will be helpful to CCM to know if they are lagging behind where others are progressing well. Often other reproductive health programs are well funded and progress on their indicators could be viewed too.

To what extent CCMs doggedly follow-up issues raised in earlier meetings is another indicator of good performance, worth looking into. This will ensure that recommendations made in previous meetings are not lost out of sight.

I think members have correctly pointed out the importance of involving beneficiaries of the programs in planning and evaluation exercises. It is as much important as is to involve people responsible to implement the programs. CCMs can ensure procedures that facilitate these approaches.

I also think it would be helpful by way of transparency to invite outside experts and knowledgeable observers to the CCM’s meetings to learn what they think and recommend.

Each CCM  has guidelines by which it operates.  There are goals and objectives and to be met which are timebound.
 
A well performing CCM is focused on meeting objectives and goals.  It sets fora for soliciting stakeholder inputs.  The members have to be knowledgable on specific subjects before they can begin to solicit input as the participation of the general public and stakeholders needs facilitation and guidance.  If the members are not knowledgable they should engage specific stakeholders to facilitate discussions. 
 
A well performing CCM has its annual agenda set and communicated early enough to allow for stakeholder participation.
