People in many countries, especially African countries affected by the HIV/AIDS and TB diseases feel shy or reluctant to come out openly to share their views or offer meaningful contribution to educative and other programs that can help prevent or alleviate the suffering of those affected by the diseases. This is unfortunate and needs to be checked. Stigmatization is the main problem.

However, it is essential that representatives of those affected by the diseases are found in the CCMs. In doing so, their feelings, opinions and sufferings will be better known, better sorted out, better analyzed and better solutions found to them.

Again, in many countries, especially African countries such as Ghana, the laws do not permit people to become homosexuals, prostitutes or inject illegal drugs. As an Assistant Superintendent of Police in Ghana, I wouldn’t entertain or encourage homosexuals and prostitutes to come out and promote their “profession”, trade or beliefs in any way. No. Not at all. Some Islamic countries such as Sudan have the Islamic Laws (Sharia Law) in place. In view of this, hardly can someone involved in this practice but affected by the disease come out openly for him/her to be condemned or arrested by law enforcement officers. Such people cannot be selected as members of the Country Coordinating Mechanisms (CCMs) because of the criminality involved.

I must admit that in Ghana, for example, some few courageous but affected people do participate in preventive and educative programs to help reduce the stigmatization and also educate people about the preventive steps people should take to avoid the diseases. It is a credit to them. They are not prostitutes or homosexuals. Some of these people can be selected or nominated to be part of the CCMs. They will be in a better position to contribute positively towards the objectives of the Global Fund. If they or their colleagues are not getting the drugs or the needed aid, they will be bold to say it because their health, life and future are at stake.

I must surely add that to serve as motivation, if some of the affected persons come out to participate in the CCMs and their programs continually, they must be motivated or encouraged in the form of reward such as per diem or allowance.

I will like to state that the issue of same sex relationship is seen as a 'taboo' in most African Countries, as a result of these, it will be so difficult to know if some of the members practice same sex affairs.

Je voudrais renouer avec e-forum après deux mois d'absence due à la situation particulière que traverse la gunée depuis la mort de son deuxième président et la prise du pouvoir par l'armée le 23 Décembre 2008.
 
Pour le sujet actuel concernant la participation des groupes affectés , je peux dire que le CCM-guinée a inclus dans son règlement intérieur les PVVIH et les associations de lutte contre la tuberculose et le paludisme.
Dans le Bureau Exécutif il y a une PVVIH qui est deuxième Vice-Président.
 
Pour ce qui est des travailleurs de sexes ( hétéro et homosexuels) il y a des programmes qui  visent la satisfaction des leurs besoins . Dans le cadre de la lutte contre les IST/VIH/SIDA, certaines ONGs et autres intervenants s'activent à leur identification, sensibilisation et référence vers des centres de prise en charge .
 
Les groupes affectés sont membres des différentes commissions de travail constituées par le CCM-Guinée.
 
Il faut dire que le CCM-Guinée a des besoins énormes en renforcement des capacités pour une meilleure implication des groupes affectés par lestrois maladies dans les différents programmes de prévention et de prise en charge.

I would like to reconnect with e-forum after two months of absence due to the special circumstances facing Guinea since the death of its second president and the military's coming to power on 23 December 2008.
 
With regards to the participation of affected populations, I can say that in its rules the CCM in Guinea has included PLWHAs and associations fighting against tuberculosis and malaria. On the Executive Board there is a PLWHA who serves as Second Vice-President.
 
For sex workers (hetero and homosexual), there are programs that meet their needs. As part of the fight against STI/HIV/AIDS, some NGOs and other stakeholders are working to identify, raise awareness and reference care centers.
 
Affected populations are members of various working committees established by the CCM in Guinea.
 
It must be said that the CCM in Guinea has big capacity building needs for greater involvement of populations affected by the three diseases in the various prevention and care programs.

TO, CCM(Participation) Regarding participation of affected population in the CCM,this statement is not an overwhelming definition of the CCM addressing functioning.The CCM(from structure standpoint)is representation of national proposal,various stakeholder,linkage,national consensus with varied CCM models in various countries.In community system strengthening(CSS),affected population component is representation of socio-economic impact of various diseases,nature of discrimination at health facilities,isolation,increase in health expenditure,non- availability of grass root empowerment,social,behavioural& moral issues,general equilibrium analysis,limited laboratory facilities, lack of proper arrangement,mandatory counselling,affordable medical treatment,constitutional article,jurisdiction,incomplete adherence, poor safety profile of treatment regimens & not selective testing for diseases.On the basis of these factors CSS could obtain participation from country's existing CCM models components.No.of CCM models have constitutional provision& accessibility from various implementers. Discussion e-forum 2008 could detail obtaining participation for affected population. The GF has international expertise health & development framework with policy ,technical issue,professional learning ,survey& research& international monetary structure for resources mobilization,disbursement of fund,funding decision& community system strengthening.Many countries do not have these formal international framework & mechanism for addressing number of major human development ecological issues & survival of essentials. At all times social,moral sensitive program communication should depend upon country's social& political accountability,commitment for socio-behavioural facts.Definitely GF has strong advocacy&improved international communication framework in the form of CCM,CSS,e-forum,for global participation,CCM could play an important strong communicative role for social,behavioural,ethical& moral issues by providing different political& social institution participation/contribution/communication/human development/participation of political activist/faith leaders for awareness,international health guidelines for affected population framework by mass campaign against number of diseases based on gender facts,global partnership for community system development& empowerment measures. The GF has strong economical advocacy framework for strategic intervention in the form of CSS,CCM models in the areas of education, & an economic opportunity for affected population could reduce risk of exploitation,inhuman approaches,& diseases. Another additional problem is that we might offer those nominated some incentives and instead of making them more dissolved in the community ,we do discriminate them  by trying to isolate them from their environment and work places.
 
We should mention also the risk of putting them as a members in the CCM as they will be at risk of violence from some extremist ,at least in our country.

In a country like Iraq, we need to address 1st the impact of populations under these categories in the strategic objectives. As many decision makers still hesitate to confess that those are exist and its not uncommon, so situation analysis relying on relevant data is important. Also mapping the location where those people are expected to be functioning is also important, so operation plans should address the activitiies needed and the best way to include them as members in the national CCM.

Participation of affected populations in the CCM

Country Coordinating Mechanisms (CCMs) are country-level private-public partnerships central to the Global Fund's commitment to local ownership and participatory decision-making. Representation in the CCM is therefore critical to efficient grant implementation and effective communication with all parties involved. Involving people affected by the diseases brings a whole new set of rules and points of view to program planning and implementation. With this in mind:

Are people from affected populations* members of your CCM?

Suggestion: your answer could include: what has the CCM done to reach out to key affected populations and to include them; challenges faced when reaching out and obtaining participation; the contribution that this group can make to the activities of the CCM. 

* Men who have sex with men, drug users, sex workers, transgender, women, girls, people affected by the three diseases, to name a few.
