To, CCM-DISCUSSION(PROPOSAL DEVELOPMENT) For inclusive proposal development,participation from various stakeholder,national consensus,linkage,community system strengthening proposal component in country could give rational inclusive proposal development process.In GF mechanism, number of CCM models have specific country based an anticipative resources allocation components.These components could include in community system strengthening proposal development process,these components could participate with LFA,Linkage,& stake holders at community level for addressing issues, evaluation& predictability of rational proposal development.Governance,social,economical,constitutional legal institution at country level could integrate with an anticipative allocation components of country CCM model for inclusive proposal development process.On the basis of rational assessment,inclusive proposal development could address specific issues,priority,allocation &mobilization of resources in countries. Regarding recommendation for inclusive proposal development,discussion e-forum GF Secretariat priority could determine priority for inclusive proposal procurement.The GF Secretariat could interact with many stakeholders in the form of research,development,civil-society,social institutions, Govt, for proposals procurement through TRP,country's TRP could collect technical priority& measures from the above institutions. The GF Secretariat could ease country's proposals mechanism for addressing specific strategy & intervention alongwith adhoc consultancies,facilitating communication,analysis of technical assistance,for allocation of TA budget in proposals procurement. To, CCM-FORUM Regarding NGO&CBO participation,evaluation& strategic review functioning for CCM addressing in country proposal procurement,NGO& CBO could integrate & participate but proposal review is a comprehensive process.A single NGO/CBO can not define above review process.A review process is a composite of methodology,analysis, allocation,application,& evaluation of complete country proposal for addressing GF mechanism in a country.Specific community based organisation& NGO in community system strengthening should get an opportunity for proposal development. If NGO/CBO gets an opportunity for review process on the basis of measures providing merit& relevant solutions providing capacity then NGO/CBO could integrate with CCM components for country proposal review& rational proposal development.Civil-society in the form of NGO/CBO could develop inclusive proposal procurement,discussion e-forum based upon inclusive structure of CCM proposal development process,could define an open& transparent process for developing proposal through medium of representation. For inclusive structure of CCM proposals development process ,civil-society is one of the constitutional medium of representing civil-rights& prevention of social liberties against undemocratic approaches,discrimination,inequality, hate rd,obscurantism,obsolete traditions,untouchable factors based on religious malpractices or by specific dominated communities.    IN ANY ORGANISATION OR INSTITUTION WHERE THERE IS TRANSPARENCY AND ACCOUNTABILITY IN ITS PRACTICAL TERMS IS ALWAYS BEING SUPPORTED.AT MOMENT I DO NOT AGREE WITH THE STATEMENT BECAUSE I DO NOT KNOW THE COMPOSITION OF THE CCMs INTERMS OF LEADERSHIP AND MEMBERS.WHO ARE THEY AND APPOINTED BY WHO?
         I THINK THE NGOs AND CBOs ARE NOT GETTING A FAIR SHOT THOUGH REPRESENTED.THIS IS BECAUSE THEY HAVE BEEB ASKING FOR EQUIPMENT eg MICROSCOPE WHICH HAS NOT BEEN SUPPLIED UP TO DATE.ONE LEADER FROM CBO WAS VOTED TO HEAD AN  INSTITUTION BUT SINCE HE COULD NOT DANCE TO THE TONE OF THE MINISTER,HE COULD NOT BE IN THAT POSITION FOR LONG.SO LET US CONSTRUCTIVELY LOOK INTO THE COMPOSITION OF THE CCM BECAUSE THEY CAN BE SINGING TRANSPARENCY AND STILL PAYING LIP SERVICES AND SATISFYING THOSE THAT WORKED THEIR WAY INTO THE CCM LEADERSHIH LEAVING THE COMMUNITY, NATION  AND THE WHOLE WORLD IN DANGER.

Personally i do not agree with this statement. Even what is now called "Open and transparent" is only so to the government and agents ,because this method is still complex to some people who are not taught how to write such form of complex proposal and not able to employ individuals or organizations that specialize in proposal writing.
 
From what has been going on, it seems the Fund considers how well one can write a complex proposal for the choice of PR and SR, and their ability to retire released funds on paper very neatly and successfully.
 
To me, the Fund does not consider if the money released has got to the target population through implementation of their objectives. It should be noted that.
 
(1)   The PR and SR are governments and their agents who are out of touch with the poor masses. How many of
      those who have been recieving these funds are really in contact with rural community residents?.
(2)   Public spirited individuals who are in contact with the rural community residents, have no technical know how of
      writing such as complex proposal.
 
Proposal writing is quite neccesary but should follow simple and general format like :
Executive Summary, Introduction - background, Justification,objectives,scope of study,relevance of study,study area
Literature review, Methodology, Budgeting and some other issues.
 
I believe the sole aim of the Fund should be now to get men and women of intergrity and probity who will be able to get to the target population, irrespective of whether they are able to write a good proposal or not.
 
This should not call CCM, if any, their role should be reviewed.
 
Placing emphasis on proposal writing is way of giving the funds to the government and the cream of the society while telling the whole world that you are helping the discriminated, stigmatized, and down trodden people of rural communities, slums and shanty areas.

Dear friends,

Regarding this week question in general, CCM now have an open and transparent process for developing proposals. Would you agree with this statement? The CCM have to close contact with the in country e-forum members in developing proposals. The country co-ordinating mechanism should be impartial, independent and antonymous. They have to call proposal through leading magazine on behalf on the Global fund Secretariat. CCM might have flexible, helping tendency and not to be a fault finders. In India, the CCM and demand of proposal. Even they not response e-mails/Fax regarding Global fund round 9 proposals. I had submitted round 9 proposal to Global fund secretariat through email. But the secretariat directed me to contact CCM India, in turn I contacted CCM India, but till time I have not receive any message from it. I do not know what strategies are adopted by the CCM India.

To, CCM-FORUM Definitely country's CCM have an open& transparent process for development of proposal from Govt,CSO,&Private sector participation, but specific country CCM structure& capacity are determining factors for an accountable & inclusive proposal development proess.Number of CCM models exists in various regions& countries i.e. public,multisector,private,civil society,UNDP,South African,NGO. These models have, in country partnership & country framework in specific country's constitutional assessment,degree & levels of an open & transparent process will depend upon CCM component's social,legal,economical,constitutional,ethical addressing of GF mechanism for community participation exists in country. Number of developing countries have still claimed non-responsive & not transparent proposal development process. Discussion e-forum, civil-society participation in proposal development process could detail social representation for proposal development process in GF mechanism. In CCM proposals development process,civil-society could give social representation of Govt,legal,economical,constitutional institutional mechanism in countries.For above development process degree of civil- society definition,structure,&function might differ in various culture religion& ethnic values based countries. Definition of civil-society could be understood by democratic values, advanced cultural & social institution addressing gender sensitive approaches,reduction of conflict,disagreement,&problematic judgement by assessment of civil rights & liberties. In CCM proposals development process,if civil-society gets an opportunity for participation than it should represent with constitutional, ethical,& formal interaction of community,groups & organisation. Civil-society participation could give rationalism in proposals development process with socio-legal structure exists in countries. Civil-society could interact with grassroot,rural,&affected population for providing civil-rights,articles,judgement,general equilibrium analysis of socio-economic indicators& factors(accessibility of products/services/resources/awareness of economical structures i.e. market,specification,&standardization,agriculture,industry,social structure including hospital,schools,college,family welfare centers, housing,NGO,welfare agencies. Excellent commentary!
 
We know that Nigeria is the recipient of a number of the Global Fund grants. The question is whether this funding is reaching communities in need and how. We are Civil Society Action Team (CSAT) that was established to help community organizations access funding from the Global Fund and educate people about HIV/AIDS and prevention methods.

We want to hear more from community organizations about their links with national NGO networks through which they can join programs funded by the GF.

CSAT Priorities
 
- Mobilize civil society organisations to engage with Global Fund opportunities
- Improve access of community organizations to technical support and capacity building
- Advocate for participation of key affected populations in HIV and AIDS programs funded by the Global Fund

I operate in Andhra Pradesh, as a Consultant for Spanish Foundation. Right now I work in 4 districts, with NGOs. Our focus has been HIV AIDS, TB AND MDR TB. I see that NGOs are not prepared to apply and seek fund. Speaking for myself, I would love to be involved in Proposal Development Process in Andhra Pradesh itself. I would like to see this happen. any takers?

REVIEW OF GUIDELINES OF PROPOSAL WRITING, CCM ROLES AND CHOICE OF PRINCIPAL RECIPIENTS. (EDITED)

 
I wish to sincerely say congratulations on your efforts to save humanity especially the developing countries of the world, which are mostly African Countries plaque by HIV/AID pandemic, TB and malaria. Through your Global Fund Programmes millions of lives are saved globally.

I know quite well that you have experts in various fields that know everything going on here in Nigeria and other African Countries than we Africans. Although this is very true, there are certain salient issues I would like to draw your attention to. Two weeks ago, I sent an enquiry requesting for submission of proposal for voluntary screening of HIV/AIDS in rural communities in Akwa Ibom State, Nigeria.

We wanted to do this in order to alleviate the sufferings of the down trodden people through testing, counseling, sensitization and referral of affected individuals to centre, where the can get treatment. We did not know that your programme also involves treatment and other services.

Your response was quite nice and okay, but I was discouraged because of bottle neck and tough bureaucracy before reaching the Nigeria’s CCM. We have a rural community base health care NGO but could not achieve much because of tough issues in accessing help. 
 
I wish you give consideration to the following point I am making:

1. Majority of Nigerians are rural dwellers and do not know nor have heard anything about Global fund programmes since they have never had contact with any health care givers.

2. Your lofty programmes only stop in urban areas where there are Teaching hospitals, Specialist hospitals and General hospitals that is where testing, counseling and treatment usually take place.

3. There are no health care facilities in almost all the rural communities. Therefore the possibility of testing, counseling, and treatment is not there, unless some certain measures are developed.

4. Globally Nigeria is ranked second to India in terms of maternal mortality rate (MMR).

5. Greater percentage of Nigerian women about 80% and above have home deliveries.

6. Based on points 4 and 5 above one can easily envisage the level of mother to child transmission (MTCT) of HIV/AIDs.

7. Lack of proper awareness and sensitization programmes at rural level. Most rural dwellers only know that there is a disease called “eight” not “AIDS” that is killing people somewhere and to them since they have never come  in contact with someone who test  positive and treated for HIV/AIDS, they claim it is the disease of the mind.

8. High incidence of teenage pregnancy, promiscuity, sexually transmitted diseases (STDs) and lack of use and proper knowledge about  condom increase the risk of contacting HIV/AIDS especially amongst young girls and boys.

9. Quackery in medical  practice thrives seriously in rural communities with the quacks using one needle to give several people injections, one can easily imagine the rate of transmission of HIV/AIDS in this circumstance.

10. Local circumcision; Greater percentage of Nigerian women have home deliveries, TBAs (traditional birth attendants) usually use the same knife to perform circumcision on different baby boys.

11. Illegal and septic abortion usually perform by quacks on different young girls using unsterile instrument, sometimes pulling out their wombs and intestines through the virgina to the outside world.

12. Incision and drainage of abscesses done locally in rural communities utilizes one and the same knife on several people.

13. I do not need to mention cutting of umbilical cord during child birth, tribal marks and scarifications made on the body as a method of treatment and several other method not mention here which are going on in rural communities.

With all these things going on in our rural communities there is no reason to doubt the report that sub-Saharan Africa is the most acutely afflicted region with HIV/AIDS.

In the area of malaria where your organization have spent millions of dollars on insecticide treated net, no rural dweller has ever heard anything about such net.

In reality, the people seriously in need of help are completely out of touch with your lofty programmes. It is the cream of the nation who can afford this net, treatment of malaria; testing and treatment of HIV/AIDS and TB are the beneficiaries, relatives/friends of those who work in these hospitals and some urban residents who have time to visit these hospitals for treatment.

Referring to “Guidelines for Proposals-round 9, section 4.3; major constraint and gaps”. I quote “the particular vulnerability of key affected population should receive particular attention in this review, as should relative capacity of non-government and government sectors to support and expand services to these populations. An important initial question to help planning may be “where do people, especially key affected populations including women, sexual minorities, currently go for health services, and do these need strengthening to save more people and to save them more effectively and efficiently?”. The 4.3.1 goes on to elaborate on “A comprehensive description of weakness and gaps,” where five points are listed.

Also under “objectives of health system strengthening”. I also quote “As discussed in s.4.3 above, this assessment should consider the broad range of health system weaknesses that affect access to services by key affected populations (including different needs of women and men, girls and boys) sexual minorities and people who are not presently visible to service delivery providers due to stigma, discrimination and other barriers to equal access.

To make your programme broad-based extending to the down trodden of the rural communities, I wish the following points should be looked into, though space will not allow me to elaborate on them.

1. The nitty-gritty of proposal writing. Majorities of public-spirited individuals can not write proposal that would meet your starndard. It is the cream of the nation who are very far away from the downtrodden of the rural communities that have the technical know how of writing proposal that will meet your standard. I believe you do this to check-make fraudsters, however, majority of fraudsters have international repute.

2. NGOs with rural community health care interest should be chosen to initiate and conduct voluntary testing, counseling, and treatment and other programmes in all the states. Where there is no such NGO in a particular state, the nearby state NGO can perform the same function. These NGOs should employ the services of medical doctors, nurses and other paramedical staff. Designated general hospital and comprehensive health centres close to rural communities should be stocked with required drugs where patients tested positive should be referred for their treatment.

3. The duty of CCM should be to coordinate, monitor and evaluate the progress recorded by these NGOs and send the report to you for consideration. Government should only come in through their representative in the CCM.

4. Dual track financing should be highly discouraged. In Nigeria there is enough money to tackle these monsters to a stand still, but because of mismanagement. And so sending this money through them creates double jeopardy for the down-drodden.

5. To record a huge success there should be proper documentation. Registers should be kept for recording of all the communities visited. The names of villages, village heads and other community leaders, local government and state clearly recorded. The names of people tested, counseled and those placed on treatment should clearly be recorded. The register should be signed or thumb-printed by the village heads and community leaders.

6. Mosquitoes net should be distributed to the communities best on number of houses but not on head count. The village heads and communities leaders should sign or thumb-print the register

7. Investigations can easily be carried out whether such exercise have taken place in such community through CCM, local government, state government or any independent body for your confirmation.

What is happening in rural communities here would have been better seen than heard for one to be able to empathize with them.

Base on the above presentation, I hereby request to submit a proposal without necessarily passing through Nigeria’s CCM to conduct a pilot study on this in Akwa Ibom State, Nigeria for your full implementation during subsequent rounds if the project is successful.

As the president of a French based NGO trying to help a Tribal ( Indigenous people) group in the Philippines through an Alternative Learning System needed because children scattered in far away hills cannot access , especially when they are too young,  the Formal schools , so, some specially trained educated members of their community are teaching,
 
as I became aware of the numerous cases of Tuberculosis in those villages , with a high mortality of young adults (children being relatively protected by vaccination) , I have proposed to the responsible M.D. of the country that I could met personally in last October Conference in Paris , that those schools and the trained Tribal members ( midwives, nurses , teachers who could study in College largely thanks to us ) could be employed in the fight against Tuberculosis as they are living precisely where many difficult to reach patients live .
 
I am surprised not to get a clear answer if this proposal is accepted ? how the collaboration could go ? what to do ?
being a new member of the IUATLD and on Global Forum may be my lack of experience is the reason , so i am asking the help of the CCM about it ; nevertheless , I have a feeling that small NGOs get a hard time to be listen to , even if the subject of Pr.Ester Duflo of the M.I.T. in College de France (www.college-de-france.fr) , in quoting and analyzing the work of small NGOs in Third World , give us hope that such down to earth experience can be given some consideration.

In general, CCMs now have an open and transparent process for developing proposals. Would you agree with this statement?

Your answer could include (but does not have to be limited to): whether you think NGOs and CBOs are getting a fair shot at having their proposals reviewed and included in a wider national proposal; any recommendations you would give to the Global Fund to make proposal development more inclusive and/or responsive to community input.
