Involving many organizations that have the capacity building ,whether human and or financial are so important from my point of view.
 
In Iraq we did share many of highly qualified organizations for the sake of technical assistance to ensure successful planning and implementation of our program.
 
We do prefer that those org. are included under one umberella to achieve the intended goals.

Dear Colleagues,

When all is said and done, the bottom line is that donors, Global Fund and the recipient countries, want to see results of the precious and scarce resources spent in eliminating the three diseases. If the recipient nations achieve adequate results, entirely on their own, so much the better. However, as we know, national capacity varies vastly among countries. Therefore, it is only prudent to make detailed assessments of each recipient organization before a grant is given or at the earliest possible. Comprehensive assessments that would include information gathering epidemiological data and national expertise in various disciplines such as behavior change communication, post-graduate specialization in respective diseases, treatment and preventive care capacity, could also serve as a baseline.  These baselines can be used to determine what kind of technical and financial inputs are available in the country and what must be brought in from international pools. Baselines can also serve as reference points for measuring subsequent progress. Assessment can be made fruitfully at any point in time even in situations where programmes have been running for several years.

Global Fund can emulate other international organization’s practice of using multi-disciplinary teams to conduct field assessments. Such teams may also advise governments of the needs identified and strategic action they might take vis-à-vis Global Fund. UNFPA and UNICEF established Country Support Teams in each region that not only carried out assessments; they also assisted countries in programme development, training, project monitoring and evaluation. The system was less expensive than engaging numerous consultants for long durations. Alternate arrangements through partnering institutions and ad-hoc teams are also possible, with some limitations.  

Needless to say that assessment exercise will identify the nature, duration, and scope of technical support needs of the country. Needs arising out of the unforeseen circumstances can be met by organizing a pool of experts such as the Global Fund’s back-up Technical Review Panel.  If Global Fund does not have a situation analysis or assessment practice already, may it be its top priority. Leaving everything to recipient countries alone is not only risky it is also unsympathetic approach. To ensure programmes succeed and results are achieved, no lacunae should be left out.

I am sending discussion e-forum 2008 based upon technical review proposals& technical evaluation reference group assessment in country's proposal development. Above assessment could ensure technical priority during GF implementation at country level.
 
In coming future Technical review proposals would be determining factors of grant agreement,funding& country's proposals mechanism for addressing specific strategy & intervention.Effective implementation of technical review proposals,GF nodal TRP& TERG should develop intra-TRP& TERG network within countries for proper functioning& appropriate resourced country co-ordinating mechanism. GF nodal TRP & TERG could provide technical assistance with international health guidelines,international standard of technical approaches,counselling,coverage of laboratory facility, application of imaging& recombinant DNA technology,invasive procedure bio-safety,genetic counselling,treatment protocol,algorithm ,to less GNP& less basic infrastructure countries.Country's TRP proposals addressing capacity would decide T.A.budget allocation in country's proposals development.Due to differ socio-technical& economical environment ,GF technical review proposals could give abasic framework for specific country's TRP & TERG for testing methodlogy,quality assurance,networking analysis,innovative financing component of GF mechanism could link with assessment of technical budget for ensuring effective implementation of TRP & TERG in less economical infrastructure countries.In future scenario technical assistance information & priority could give capacity development support for country's technical review proposals. -
 
In preparation of country proposals & implementation of program GF capacity development support component should involve national health system including public & private ,independent evaluation. Representation of above sectors might differ in evaluation,standards & statistical approaches.In developing countries ,evaluation degree of health services research approaches including bio-medical research, intersectoral research,could be determining factors of capacity development supportof technical assistance. In GF framework document,GF principles have included country's proposals for international health financing,high quality proposals formation is not possible in less GNP ,less awareness of technology, less resources,countries in implementation of GF program. For proposals development technical assistance could give technical priority associated with available resources.Technical assistance might differ & more detailed in access of health services across the social.economical,& geographical expanse of many developing countries.Possible obstacles in the form of (lack of quantitative- methods) including cost-effective models ,lack of network& system analysis e.g. GF program evaluation& review technique could expand needs of sub-recipient technical assistance. In the above condition TERG(Technical evaluation reference group) can expand its own role by providing testing methodlogy,measurement for quality assurance,network analysis. For technical capacity strengthening I have sent number of country's proposals since 2003 to WHO& GlobalFund. -Application therapy -Retinitis pigmentosa -International authorization -Non-tuberculous mycobecteria -Screening List of proposals & mechanism 2004,2005,2006,2007,2008,have been submitted to GF forum& proposal team. These proposals were based upon analysis of highly critical factors for capacity development & analysis of possible obstacles in preparation of country's proposals. These proposals have given better predictability of GF partnership strategies& formal relationship to affected population in South-East Asia. Thank you

Regarding this New Year question of “how best can CCMs ensure effective coordination of technical support during grant implementation at country level?

I recommend the following strategies to adopt to undertake better actions towards to implement the global fund programmes.

The CCM have to ensure the PRs & SPRs capability and track record of themselves.

The CCM have to interact with the partners in all spheres

The CCM have to prepare time frme to implement the glopal fund programs

The hev to establish good & fair formalities about to implement the global fund programs

The CCM have to prepare and distribute a clear cut publicities, IEC materials, advertishnent in support of the global fund’s program.

The CCM have to plan the programs which have to be implemented by the PRs& SPRs in proper manner.

The CCM have to prepare information education and communication materials as related to the local langhages

The CCM should have to visit the implementing agencies closely and advise them to following the proper management and improve the project indicators.

The CCM have to conduct social related programs through the PRs/SPRs as Per the Local Functions and Other Distend Periods.

The CCM mainly arrange to conduct the global fund’s programs if necessary through technical needs ie, resource persons, lab technician, outreach workers, counselors, peer educators, and etc,

The CCM can involve to train the out reach workers, peer educators and staff members effectively and periodically as per the direction given by the global fund.

The CCM have to implement the global fund’s implementation program through various technical and oral indicators as per global fund’s secretariat norms.

The CCM to supply all information, education & communication materials as per the targeted programs in advance to the implementing agencies

The CCM have to transperate and act as per the global funds quidlines it has to inform all related matters to each part without delay or handedness

In view of there above facts, I suggest that the CCM is one among the partner of the PRs/ SPRs in implementing the global funds program to fight against HIV/AIDS/malaria/tuber glucoses.

Happy New Year to each one of the members and coordinators in a special way.
GF CCMs would ensure appropriate technical support by:
  1. Ensuring that the CCF formation is representative enough and it includes members from various technical expertise are.I did mention last year that we need the CSOs representatives(who must be experts or persons dealing or coordinationg the priority areas in their organizations-HIV(Prevention,Treatment,Care,Support and Mitigation),TB or Malaria.It should also include independent consultants who are experts in those priority areas-doctors,community Health TOTs,capacity builders etc,representatives from the GF and other like minded CSOs among others.Community representatives should be included to ensure that the grassroots are reached.This will ensure effective and efficient implementation.
  2. Having continouos evaluations and good M&E systems in place,with good and user frienly tools...have exteranal consultancies (outsorce) to ensure objectivity...this forum has lots of consultans and experts on this and their expertise could be tapped.
  3. CCMs should be more proactive...and participatory...this will ensure good implementation,will yield good results and will have long term impact,both qualitative and quantitative.
The support should be long term if we have to realize impact.Quartely monitoring,mid term evaluations and end term evaluations.These technical experts could be invited to attend the quaterly monitoring meetings by the GF partners so as to hear the progress in the implementation from the implementers themselves as this would help them identify the variances and put activities back on course or re direct time,energy and resources to achieve good impact.

Happy new year myglobalfund collegues.

We are in 2009 and what vows have we made for the challenges of the three  disease that are within our midst that is for HIV, TB  and malaria. I made a vow that as Koch we will together stop TB, and said it is simply to stop TB if we follow the correct strategies and guideline that we have. What is left is commitment in what we do.
 
I see that you have asked the question how best can ccms ensure effective co-ordination of technical support during grant implementation at country level. Mmmm..... I have been receiving technical support for the past three years from two big organisation that work closely with tuberculosis programs. This is my experience, I am not writing fiction but that what I actual experienced. I was physical involved doing 85% of the technical support work in the NTP national reference laboratory and its tuberculosis laboratory network. I never saw the ccm I all had about it now towards the second quarter of 2008 when we were winding up the programs and those who were saying whatever was done should done by they select. I would say with the technical support for those three years we had shaped the tuberculosis program and it was doing very well, not knowing whether I am saying the ccm does not have much it what is is happening with the program except when projects and programs are submitting proposal after that they actual do not follow what is happening whether implementation has been done or not from my observation I think it is not for them to worry.
 
How best for them to ensure effective co-ordination can they please be physical involved and make sure the ccm has the right people who have a vast knowledge of each program and what is required including in proper ordering of reagents, drugs consumables and equipment. With this even know how, how technical support is handled or done. We need a ccm that is not political affliated but an independent ccm that has a patient at heart, not looking at funding  but looking at what should be achieved and also employ those who will produce not nepotism.

I am not a member of the CCM, in fact I work for a facility that provides technical assistance to CCMs. My comments and suggesions will therefore be informed by my direct work CCMs, PRs and other partners in grant receiving contries. Although CCMs are viewed as boards of directors; generally, however, their relationship with PRs does not suggest or reflect this governanc relationship. The mobilisation of technical assistance by the CCMs will be dependent on its relationship with the PRs and other key stakeholders like Ministries of Health, National AIDS Councils etc. Essentially he way CCMs manage these relationships will lead to a construct that ensures oversight of the grant from "craddle to grave". This is necessary because the identificaion of TA needs can happen at different layers of institutinal and strategic relationships.  If for instance the CCM is not involved in the strategic and opertaional planning stratas of the national response; it may find that TA needs would have been identified at this level. It herefore would need to develop a strategic perspective of relationships and clear understanding of the systems that are used by different stakeholders involved in the planning and impementation of programmes in the three diseases. Within the grant cycle the relationship with the PRs and other stakeholders within the context of the CCM's oversight role would define the success o faliure by the CCMs in this regard.

As a funding mechanism the Global Fund does not implement the programs it finances, nor does it provide technical assistance. Instead, the Global Fund relies on country level partners and stakeholders to provide such support to grantees. Bearing this in mind, within your CCM:

Q. How best can CCMs ensure effective coordination of technical support during grant implementation at country level?
 
Your answer could include (but does not have to be limited to): How can technical assistance be tracked and evaluated? Who should provide it? Should support be short or long term?
