Dear All, By now alot of usefull comments and inputs have been voiced out by both experts in the various fields and benefiaries of Global funds. With regards to areas of priority, it will be out of place to say either of the illments shoiud be given priority or the other should receive less attention. Reason is the intencity and gravity of iether HIV,Malaria and TB are felt differently in diffrent countries. each country is peculiar with its problems. In some all the 3 deadly disease are present. My view is Global Fund should strategise a mechanism that will allow its funding to be diversified so as to cater for all the diseases its focusing, and ensure the intervention are result oriented, so that whatever penny that is spent will bring back value for money and possitive impact in the lives of the people. Priority can be given to segment of the population that is mostly affected e.g women, children or men.No segment should be neglected, because it may have adverse consequence in the near future. I will talk about HIV & AIDS and TB because I work i country with the largest number of PLHIV in the sub Saharan Africa and whose TB infections have gone up in te last few years. I would like to mention that the roll out of ART has been a blessing,the figures are such that there are more women than men on the therapy,but the prevalence rate is still going up.However,the impact of HIV&AIDS and TB on women is overwhelming.Not only are they victims of of poverty and male domination(poverty leads them into transactional and trans-generational sex to survive and male domination is such that men can have multiple sexual partners and in some cultures it is the men who should initiate use of condom),but also shoulder the burden of care of family and community members.It is the women and girls(well and or sick)who provide care. Attention needs to be focused on both men and women and the burden of care shared with men,poverty addressed and gender equity in Prevention,Treatment,Care and Support. The civil society should participate at all levels of projects life cycle including proposal development and M&E if we have to make maningfull impact because it is them who work in and with the communities at grassroots level.Strenghtened accountability to the communities would add great value. I would like to add that governments,communities and the civil society should strengthen the monitoring and follow up on treatment protocal(both ART and TB)because some PLHIV and TB patients skip and default on medication which has increased the number of TB and AIDS related deaths in the country Best Regards to you all

PRIORITY  ISSUES
 
  • The contentious discussion on whether the GF   should concentrate more on men than women now that 56% of women are on ARVs
  • .Civil society participation in proposal development processes inclusive of categories e.g PLHAs, marginalized groups and affected populations.
  • Development of a more colloborative and coordinating systems i.e the need to address on how he secretariat should interact with stakeholders .It was a popular voice that there is need for establishment  of GF regional secretariat offices foe effective communications,evaluation of programmes and reduction of costs of maintaining the secretariat responsibilities

Dear all e-forum Members,

We have discussed many important topics of global funds can address better while carryout the project for the benefit of the target population to fight against HIV/AIDS, TB and Malaria This weeks question is what issues that have been discussed during the e-forum 2008 should the Global fund address as a priority.

As Topics like demand Implementations Partnerships and gender are all good theme and very much important t implement a projects like prevention & contort measures of HIV/AIDS, TB and malaria. Demand have create many new thinking to wards the HIV/AIDS, But many members focused on HIV/AIDS, a few members alone to pay concentrations including TB and malaria Regarding implementation all members expressed very good manner at the same time imparted about their own environment Government & private policies. But Global fund partners have to adopt the guide Hines of Global fund only. the theme partnership not much impressed, beans the local partners may not adopt the Global funds guide lines one among the discussed theme of gender strategy founds is the priority area that the Global fund have to address as a priority During this discussion, many find of out comes were observed which related to mentality undeveloped culture , is literacy, male domination etc. in all area in Global level this reselect, men are doing the production learn to run the family only, but the women are doing production, rearing children rearing cal true etc. the women suffering more than the men and ignored by the society. lesser awareness and itinerancy the women have to depend on their family / husband / relatives for diseases my option the gender theme have to pay more priority among the topics which we have discussed during the –e-forum 2008.

Received (issues& priority) discusion.From priority standpoint
GF should consider socio-economic expectation of countries.Many
developing African,Asian & less GDP countries are not familiar
with functioning & accessibility of GF mechanism.Expended
structure of country CCM, CSS capacity building & addressing intervention, innovative financing mechanism for future funding
mechanism,GF Secretariat& TRP co-operation in less health In-
frastructure countries,GF economic model for product&services based intervention in less GDP countries ,could be considered in GF
addressing priority.Discussion, Gender equality strategy could
give CSS priority in GF mechanism.

Draft of the Gender equality strategy(GF)

Received above draft issue. GF's country co-ordinating mechanism
has complete components for linking country's community structure
to formal international institutional framework.Community structure
including affected population,gender fact, health system strengthening
could address social equity, equitable access of treatment& services
in specific country for specific intervention.For specific gender
intervention,country's community understanding should take an
ethical approaches(social,economical,health,educational,empowerment)
according to country's constitution,GF has formulated five strategic
objectives for addressing in equalities in country program.For
ensuring equitable access of treatment & services in many countries
GF's community system strengthening& CCM should analyse social&
constitutional environment in specific country for gender equality
strategy.
For ensuring equality strategical concept,partner country should
ensure social & constitutional equity for women & girls by presenting
& formation of gender sensitive social institution,female literacy
rate,socio-economic indicator for gender empowerment,gender basic
employment including income,occupation associated with basic health
infrastructure& provision of health expenditure.Middle income &
less GDP country could develop better country community system for
gender equality by inclusion of GF strategic objective & universal
coverage concept& approaches.Possible obstacle could be occur ed in
implementation of above strategic objective for gender equality in
less GDP,less natural& economical resources,bigotry faith based
countries where social& country's own cultural intervention does not support above gender equality concept.
Number of social reasons could be involved in gender disparity e.g
specific culture which allows specific gender (men) for employment,
work ,right of natural& geographical resources.In the above social
economical,& cultural environment women & girls are more susceptible
to number of diseases linked with malnutrition,physiological &
psychological factors,acquired,genetically,occupational hazard,less
equipped maternity services.
GF's CSS& CCM could include grassroot& rural women empowerment
component for addressing equitable access of treatment& services
in above countries with integration of country's constitution
feasibility.
Without availability of capable grass root & rural social institution
&basic infrastructure (health,education,nutrition)specific gender
(women,girls) can not address social/economical/cultural needs to
country's community system.GF's country system strengthening should
integrate with country's specific community addressing structure,&
community proposals by providing capacity development support including governance policy & procedure,funding decision,procurement
of goods/products/services/social institution/community institution
for gender sensitive social equity.Discussion effective intervention
could give country proposal procurement based on gender sensitive
social equity .

is civil society* being given a fair opportunity to participate in the CCM proposal development process? What could the Global Fund do to encourage countries to make this process more inclusive? How can the Global Fund better support civil society organizations that have been chosen as Principal Recipients by the CCM* in their newly required responsibilities? With the majority of people on antiretroviral treatment being women (56% at the end of 2007), should the Global Fund's gender strategy focus more on men rather than concentrating on women and girls? What mechanisms could the Global Fund put in place to enhance collaboration, communication and coordination at the regional level?

Dear All, When the whole world is facing lot of problems due to Global Warming, Financial Crisis,Terrorism,Floods,Tsunami etc ,we have to look for the real priority in our targetted goals for achievement.I would like to deal with one by one:- HIV:- A Viral Disease. As we all are aware there is no direct drug to readily clear this infection.It is an herculean task to overcome this menace.Even then we have to compliment countries like UGANDA who have brought down the rates from 30% to mere 8% & it is a great achivement & we have to compliment all the team members involved in that.Such efforts are needed all over the world to put up an effective operational plan. Tuberculosis:- A Bacterial Disease. There are potential drugs available.But this bacteria is so potent that it is causing maximum damage in the underdeveloped countries & to an extent in developing countries as well.The bacteria becomes resistant every now & then for the antibiotics.There are cases reported from developed countries also & it is still an enigma. Malaria:- A Protozoan Disease. We have got adequate measures like drug therapy & vector control measures to stop the transmission.When WHO initially started the programme it was aiming at ERADICATION & realised that it is impossible because of many factors & main being the non availibility of a potent Vaccine which is ever elusive & difficult.Still there are more than 3 million deaths reported due to Malaria & mostly young ones. I feel the priority should be for this disease & maily to stop deaths due to "Malaria" & it is achievable. Our strategy should be to get a situation with "Malaria without Mortality" & towards this goal we have to work.We plan strategies for early detection & prompt treatment. Also there is a need to deciphor the reasons for complications caused due to malaria & that results in death ultimately either due to Pf or even in Pv. Hence my priority is for "Malaria without Mortality".How we achieve it needs to be worked out? Best Regards, Dr.Chazhoor John Babu, Mumbai, India

Hi all I would say currently Global Fund need to enhance participation of the civil society and private sector as these are the implimenters of the most programs. This should be the priority as of now

The GF should focus more on implementation of programmes and their impact on the beneficiaries.More often that not,some gorvenments have colluded with some INGOs to use the funds for own interests at the detriment of the intended beneficiaries.The end result has been deepened vulnerability of the poor while the implementing agencies write very good touching reports for no activities carried out,little or no impact and remain uncountable to the communities in which they operate.This issue should be addressed if meaningfull impact and susutainability of programmes has to be realized.

My comment as a priority is as follows. In most of the cases governmnts are PR or very few selected INGOs as PR. When it comes to the implemntation there are very limited SRs. At the end of the date the fund has built the capacity of few groups and the sustainablity will be questianable especially for community related interventions!. Therfore, i wish if Global Fund has some regular tracking mechansim and a must crtreia for PRs in working with CBOS, LNGOs, FBOs, civic socities and the realted bodies who will remain with the community for a sustainality.
