    Regarding effective health & social intervention linking with HIV/AIDS & sexual reproductive health, awareness, supporting & understanding of bio-behavioural change process and its integration with general health protection measures can give effective delivery of health services.
    Screening of population group& an individual including  blood-transfusion ,epidemiological survey,pre-operative testing, serological techniques,voluntary testing, specific prophylaxix, microbicides,generic anti-retroviral therapy,prevention of haematological abnormalities, surveillance system,biometric accessment,use of clean syringes,OST,PCR methods,immunisation services are effective relevant preventive health interventions  links with HIV/AIDS.

    Regarding social intervention link with HIV/AIDS &sexual reproductive health understanding of bio-behavioural change process,social
    interventions into all development actvity are essential. Social intervention should be focussed on high risk behaviour, prevention methods, abstinence,family health,local conditions, prevention of community cultural social discrimination,religious beliefs,legal prohibition on abnormal trade/cultural practices, high risk category of AIDS,capacity building for biological & cultural support for prevention of malpractices in religious beliefs.
    Global fund should give regulatory ,financial,cost effective,innovative,collaborative support in clean space mechanism, clean development mechanism ,international standards of quality for humanitarian approach,prevention of commercialization of opiate dependence, communication,technology,& indigenous media.

    Below is the my answer to the asked questions.

    Question. What are thé Most effective health and social interventions that link HIV/AIDS and sexual and reproductive health? Which ones should be supported by the Global Fund and which ones should not?
    Sexual and reproductive health care represents an opportunity to expand HIV prevention and care for women. Similarly, services provided within HIV programs provide a potential platform for sexual and reproductive health care, such as prevention of sexually transmitted infections and family planning.

    • Affirmative action should be put in place to increase éducation of girls in médical related course and subjects.
    • Improved education of the girl child
    • Improved nutrition so as to help people to couple with HIV/AIDS
    • Increase health care facilities in a bid to increase accessibility and affordability.
    • Conduct massive sensitization on women rights in order to break the strings attached to culture that marginalize women in access to health care services.
    • Sensitize men in gender related issues in order to break patriachism in society.
    • Economic empowerment of women
    • Incorporate gender and women studies on the education curriculum of health education institutions.
    The following should be funded by the global fund • Improved nutrition so as to help people to couple with HIV/AIDS.

    Training and intervention, not just focusing on those who are educated, but specifically focusing on those who are not educated, and are in rural areas.

    Much education is done in urban areas, schools, colleges and universities.
    Even companies are now exposing their workers to regular information sessions.  Unfortunately areas such as farms, rural areas, where there are truck stops, and people are not that well educated seems to have been forgotten.

    It is easy for many NGO's, NPO's, CBO's etc to work in the urban areas, but when working in rural areas and farms it is a totally different ball game.

    One is forced to do a skills, education, and health audit before embarking on any programme as the need changes from community to community.  It is important to keep in mind that visual aids play an important part in communities who have a great number of people who have little or no education.  Training manuals and visual aids have to be done in Afrikaans as well, as this is the language spoken in many of the rural areas.

    It is important to note that, in many communities, women play a major role in any interventions.  Most of the education and training offered are attended by women, and this should be kept in mind when embarking on training sessions.

    The G8 leaders made a commitment to universal access to treatment, care and prevention by 2010, and this is barely two years away, and the unmet need to universal access is hindered by lack of access to Anti Retroviral drugs/ medications for the opportunistic illnesses, the brain drain of trained medical professionals and poorly equipped health facilities to carry out testing for HIVAIDS coupled by burden caused by HIVAIDS like orphans who have to fend for themselves.
     
    I foster 45 orphans from 1991 and to graduate this year in Law and Social Worl, meanwhile another two graduate nexyt year in Medicine and usiness Statistics. Majority of the children are in high school and primary level. One of the kids who is going to the Seminary once told a crowd that , " If it was not for my new mother I could have been a chicken thief in the village or even died, but I am now a respected person standing to address all of you from the pulpit". Many of these kids need a home and a family they can relate to, they need to be loved and nutured as normal children.
     
    Culture does not favour women and the girl children, they are expected to get married and be submissive to the sexual demands of their partners, so the married women are getting infected in the house so they need skills to negotiate with thje husbands and microbicides to stop getting infected. I met one Doctor Herman Steven who invented a micobicidal gel which was clinically tested to be very effective and has no support to manufacture this product that could save so many lives in Africa and the world. He also invented anti retroviral drug which is very effective and why cant the global fund support such initiatives?
     
    More studies need to be conducted to investigate the role of culture in the spread of HIV/AIDS. Women can benefit from educational programs through Fuctional Adult Literacy Classes. Economic empowerment of women and youth can dramatically reduce the number of women who indulge in sex work.
     
    Finally peaceful and nonviolent means of resolving conflicts must be promoted as perpetrators use rape as a tool during conflict and often spread HIVAIDS.
     
    PLEASE GET INTOUCH WITH DR HERMAN AND FUND THIS MIRACLE ANTI RETROVIRAL DRUG AND MICROBICIDE THAT CAN SAVE THE LIVES OF MANY WOMEN IN THE WORLD.

    Action points.
    • Country  regulatory, laws  and denayal of certain rights at the community, household levels.
    • Lack of  awareness of the community members in handling issues related to sexual minorities.

    many useful suggestions have been made. One important goal is pulling together of evidence on what works in a more scaled up way - there seem to be a lot of excellent programmes - how do they fare on a bigger scale? what would be needed to make them work? Some neglected areas are 1. programmes and services to prevent and treat the outcomes of intimate partner violence (there is an excellent public sector health initiative in Kenya on this which provides routine HIV prophylaxis) Violence can be a factor in HIV transmission. 2. Male sexual health and HIV. This is a really neglected area - men have few places to go with sexual health concerns (which are often psycho-social), and yet health male sexuality can play a really important role in prevention. 3. HIV and SRH educational programmes for younger adolescents (should ideally be starting by 10 or possibly even younger). it is this upcoming generation that badly needs the tools to protect itself, and data suggest that the age of sexual debut is extremely low in many countries. But this is a really hard one and so gets ignored. but it would be good to encourage creative thinking in this area and looking at what is effective.

    I think that supporting NGO's that have as main goal to improve the condition of the women as a whole can be very effective. These organizations can reach large parts of their communities and are well respected. They have in many cases enough influence to change the social conditions of some communities, by teaching women professional skills that allow them some level of economic self-sufficiency. Once this starts to happen, women gain respect; they can then start challenging traditional sexual practices and change their partner behaviours. They can be encouraged to create specific sub-groups to address HIV prevention, and provide education. These groups can also develop specific educational programs for teenage girls. In general, I agree with one of the participants of this forum that programs that are holistic in nature are more effective and sustainable.

    What are the most effective health and social interventions that link HIV/AIDS and sexual and reproductive health? Which ones should be supported by the Global Fund and which ones should not?

    Any program is effective only if it has multi facets accommodating an integrated approach for a holistic development. Always, HIV/AIDS interventions can be looked as a part of integrated health care services that enhance sexual and eproductive health. Sexual and reproductive health care begins from childhood and runs through one’s adolescence period and become hectic during adulthood. A person needs attention right through these periods to improve his/her reproductive health.

    An altogether independent and  individual health care program/ intervention would not bring in desired impact among the population to prevent HIV.  Two or three programs aiming at the community’s or populations’  multiple needs can be combined and integrated to the desired level. For example, education and health can gotogether among the children that can increase both the knowledge and health seeking behavior of the children in preventing HIV or promoting the sexual and  reproductive health. Interventions on life skills will be a major cementing factor here. Children at secondary, higher secondary, college level woudl get benefits  - where parents are also available to participate. 

    Other possible multi sectoral interventions that would be most effective are :

    Health and Benefit schemes – through community structures – stregthening the local bodies – SHGs/ panchayats / community associations

    Both the school and non school where families and local leadership are available 

    Repoductive health and HIV prevetion through workplace models -

    Mostly unorganised and migrant population 

    Such programs also help the organisors to broad base their access to resources both physical and financial.
     

    Which one should not ?

    Specific targetted  interventions do not have seeds of sustainability. Similarly direct service interventions are also prohibitive. Similarly, the interventions that aim at strengthening the capacities and skills devoid of professionals’  support would also be less effective.

     Like always said HIV/AIDS is no longer only a health concern but also socio-economic concern; in Uganda HIV/AIDS is a political concern. Gender in HIV/AIDS initiatives/responses, can and should be looked at in two ways, as mandatory requirement and two as a demand driven response.
    Only institutions with a clear gender policy or guidelines or strategically take into consideration reaching gender issues should be funded
     
    Gender issues have been misconceptualized to be women issues, but most agreeable is the fact that women and men issues can not be separated and that to reach to men there is a role women play and vise viser. Organizations with clear strategic approaches to reach to a particular sex with with considerations of what the other sex have to play so as to intervene in a gender related issue are the best approaches.
    Today, the best organizations to fund are those that are either a part of a holistic network, or providing a holistic approach.  However, in providing this holistic approach the most vulnerable groups are a target, and these target groups are mainly xterised by social and economic underminings. Which in Africa either define or promote gender differences.
     
    Projects established by OR affiliated to religious bodies are one of the most effective projects of reaching out to populations with HIV prevalence; most especially those religions that promote fidelity and gender considerations, when intervening in prevention and social support.
    Religious institutions have in most cases a well defined institutional arrangements that give their projects a comparative edge over others in reaching out to the family as the best start for gender and HIV/AIDS interventions. Reaching out to the family as a corner stone in doctrining gender issues is a strategic approach and therefore I will recommend that all countries initiate an inter-religious body.

     In Tanzania one of the major challenges to HIV and AIDS as far as sexual and reproductive health issues are concerned is the ways in which women and girls have no control over their sexuality and hence reproductive rights. Due to high lelevs of patriarchal tendencies, women and girls cannot negotiate for sex. hence whatever is being done is for the mens' pleasure and the benefits of others as far as her sexuality is concerned. In order tto then intervene and make a meangful discussions on this, the first inteverntion could be empowerin both men and women to understand the sexual and reproductive rights. In other words creating awareness.
     It can also be extended to deliberate efforts by the proposal to require to shoe with data the number of women and girls who not only will they be reached but have knowledge about their sexual and repordcutive rights.
     There is still a very big gap on how to program issues of sexuality and reproductive rights in the proposal. A clear analysis should be provided to the team at all levels on how to maistream gender into the proposal. It is only through this process that theinterentons of sexuality and reproductive rights will be determined.

    The effective health and social interventions that link HIV/AIDS and sexual and reproductive health could be look at in the following topics
     
    - To initiate, organize and sponsor training programs and courses in the fields of primary health care, home economics, etc.
    - To instill and develop mental, technical and health skills and capacities building in health personnel in the prevention, diagnosis, management and care of HIV/AIDS infected and affected persons. Also to act to inform and sensitized communities on their health needs.
    - Further sponsor is needed to PROVIDING PSYCHOSOCIAL AND MEDICAL SUPPORT TO PLWHA & OVC THROUGH HOME-BASED CARE, This is because PLWHA and OVC suffer from reduced care and increased stigma caused by tradition and myths attached to the pandemic. The belief that the disease is a misfortune and the high practice of polygamy especially by the Fons (traditional rulers) who serve as local role models. It shall implicate local health personnel in anti stigmatization and anti discrimination behavioral change and build on their experiences.
    - Educate traditional rulers and their close aids on their responsibilities as local role models and the dangers of stigmatization of PLWHA and OVC encouraging them to serve as support service providers.
    - Organizing People Living With HIV/AIDS into functional groups.
    - Psychosocial support shall be geared towards improving the mental stability of the infected persons.
    - Training and support health care system will help care providers respond to physical and emotional and security needs of abused HIV/AIDS infected and affected persons in general.
     
    Let me at this point give a brief introduction of our organization below. Cameroon Rural Integrated Development Programme (CRIDP) is a non-political, non-profit making, non-denominational and Non Governmental Organization (NGO). This body acts to complement the efforts of Government and other non-governmental bodies in providing health care services to the under privileged members of the rural communities in order to improve on the health status and the socio-economic activities of these people.
     
    You will agree with us that about 70% of people living in the rural areas, do not have access to basic health facilities as Government cannot single handedly provide or meet their health needs. Meeting the health needs of rural populations of Cameroon is a cause for concern on the part of selfless and philanthropic individuals and organizations. It is in this attempt to meet the rural health care needs and other challenges that Cameroon Rural Integrated Development Programme (CRIDP) came into existence.

    Dear all
    I am fully aware of the fact that The Global Fund is distributing funds throughout the world. However I would like to see that emphasis should be put on Education, Training and Development here in Bangladesh. The global fund should maintain a close working relationship with grassroots organization and small local NGO's which working through Community Participation and working to create awareness in grassroots level to build on their views/expertise, to promote peer learning and exchange and this process has proven to be a source of women's empowerment.
     
    It would be highly appreciated by creating a safe environment for women & children, less-privileged communities gives them opportunities to share, find solutions to their major problems. As a result it helps to address many different issues that women face, confronting such problems collectively also reduces stigma and discrimination by giving the victims an opportunity to contribute and becoming agents of change.

    In the global phenomenon on the most vulnerable socially marginalized people, especially youth and adolescents are the alarming area of infection and links with HIV/AIDS. THE third world countries are the most affected specially the Sub-Saharan and Asian Region. THE youth and adolescents out of school, married early, early pregnant, street children, orphans, migrants, child soldiers, refugees, drug users, gang members and adolescent sex workers are the most vulnerable and socially marginalized and the ignorance are the main cause of infection on HIV/AIDS.
     
    THE governments have its capacity to bear expenditure of treatment and awareness mechanism of their own fund. A budgetary allotment and programme can make it success. THEY can impose taxes to the people of their nation to allocate fund. THE governments are required their “WILL”. BUT the Health Care units in private capacities or the non-profit making, non-governmental organizations or civil society have no such source of fund to continue their programmme of awareness and treatment. AN effective programme of allocation of GLOBAL FUND will be to sanction funds on PROJECT PROPOSALS directly or by their honest agency.

    Thank you for the opportunity to voice my opinion.

    Here at NURTURE HIV/AIDS SERVICES, we believe that prevention is one the first steps in combating this HIV/AIDS pandemic.
    The corner stone for prevention is Education, Training and Development of those living with and effected by HIV/AIDS. I am working in the HIV/AIDS "field" for the past eight years. I’ve seen what difference education could bring.

    I am fully aware of the fact that The Global Fund is distributing funds throughout the world. However, I would like to see that emphasis should be put on Education, Training and Development here in South Africa. It is amazing what simple questions a person asks during training sessions.
    Noting that little is being done to voice the collective interests and dilemma of People Living with HIV and AIDS, we agree to form an organization to address the above-mentioned issues.

    South Africa is amongst the leading countries in the world in terms of HIV/AIDS infections with a high rate of mother to child transmission and violations of fundamental human rights of the infected and affected.
    NURTURE HIV/AIDS SERVICES prides itself in its ability to meet client’s needs and add value. Our in-house trainers, as well as associate trainers, are all well experienced in field of HIV/AIDS.

    In my view most effective health and social interventions that link HIV/AIDS / sexual and Reproductive health, is to support programmes that are of a holistic nature.  Creating a safe environment for women gives them opportunities to share, find solutions to their problems.  And if a programme is of a holistic nature, it helps to address many different issues that women face, confronting such problems collectively also reduces stigma and discrimination by giving the victims an opportunity to contribute and becoming agents of change.  The global fund should maintain a close working relationship with women's grassroot organisation, build on their views/expertise, to promote peer learning and exchange and this process has proven to be a source of women's empowernment.
